摘 要:目的探讨高血压病患者的动态血压与体质指数(BMI)的关系。方法应用Medilog-DX型全自动血压监测仪计录168例原发性高血压病患者的动态血压参数;人工测得腰围、臀围、大腿周径和上臂周径等指标,并计算BMI(体重/身高^2)、腰/臀围比值(WHR)。结果24h收缩压(24h SBP)、白天收缩压(dSBP)、夜间收缩压(nSBP)与体重、BMI、腰围、WHR呈正相关(r=0.21~0.36,P〈0.05),与臀围呈负相关(r=-0.19,P〈0.05)。夜间舒张压(nDBP)与腰围呈负相关(r=-0.21,P〈0.05)。24h舒张压(24h DBP)、白天舒张压(dDBP)与年龄呈负相关(r=-0.20,-0.19,P〈0.05)。WHR和-SBP,nSBP及24hSBP水平与年。龄相关性最强(r=0.33,0.26,0.31,P〈0.01);年龄与24h DBP,nDBP水平相关性最强(r=-0.22,0.24,P〈0.05),而BMI与dDBP和24hDBP水平间相关性最强(r=0.35,0.31,均为P〈0.01)。平均血压与身高、上臂围、大腿围和上臂围/大腿围比值间无相关性。结论高血压患者昼夜血压变化与人体脂肪分布关系密切,应合理控制饮食,减少脂肪在体内的分布。Objective To investigate the correlation property of dynamic blood pressure and BMI for patients with high blood pressure. Methods The dynamic ambulatory blood pressure monitoring of 168 patients with primary hypertension was performed by Medilong-DX automatic blood pressure monitors. The waistline, buttockline, upper armline and thighline,were measured and the BMI( kg/m^2) , the ratio of waist to buttock and the ratio of upper armline were calculated. Results The 24h systolic blood pressure ( 24h SBP ) , daytime systolic blood pressure (dSBP) , nocturnal systolic blood pressure ( nSBP) showed the positive correlation with weight, BMI, waistline and WHR (r = 0. 21 -0. 36, P 〈 0.05 ) and negative correlation with buttockline. The night diastolic blood pressure (nDBP) present the negative correlation with waistline ( r = - 0. 2 1, P 〈 0.05 ). 24h diastolic blood pressure ( 24h DBP) and day diastolic blood pressure { dDBP) present the negative correlation with waistline ( r = - 0. 20, - 0. 19, P 〈 0.05 ). The WHR, dSBP, nSBP ,24h SBP were highly correlated with age ( r = 0. 33, 0. 26,0. 31, P 〈 0.01 ) ;The age was highly correlated with 24h DBP and nDBP (r = -0. 22,0. 24, P 〈 0. 051 ,The BMI present a high correlation with dDBP,24hDBP ( r = 0. 35,0. 31, P 〈 0.01 ). The average blood pressure is not correlated with height, upper armline, thighline, ration of upper armline to thighline. Conclusion The circadian rhythms of blood pressure were highly correlated with the BMI and fat distribution for patients with high blood pressure, hypertensive patients should be on diet reasonably and decrease fat distribution in the body.
