全胃切除D4术联合多脏器切除治疗进展期胃癌的临床研究

摘　　要：目的探讨全胃切除D4术联合多脏器切除治疗进展期胃癌的效果。方法观察32例进展期胃癌病人行全胃切除D4术联合多脏器切除的效果,并与姑息性手术比较。结果全胃切除D4术联合多脏器切除组患者与姑息性手术组比较,生存期较长(P<0.01),手术并发症及死亡率无明显增加。结论全胃切除D4术联合多脏器切除对部分进展期胃癌患者安全可行,可明显延长患者的生存期。Objective To study the effect of total gastrectomy D4 operation associated with multiorgan ablation to therapy progressive gastric carcinoma. Methods The effect of 32 cases of progressive gastric carcinoma accepted D4 operation associated with muhiorgan ablation were observed to compare with the effect of palliative surgery. Results Compared with palliative surgery, total gastrectomy D4 operation associated with multiorgan ablation gained patients a longer life span（P 〈 0.01 ）. There were no significant difference of complications of operation and the death rate between them. Conclusion Total gastrectomy D4 operation associated with multiorgan ablation was safe for the patients with progressive gastric carcinoma and prolonged the life span obviously.
