外伤性绞窄性膈疝手术切口改进的治疗

摘　　要：为获得良好的外伤性绞窄性膈疝手术视野，减少脏器的损伤便于疝内容物的还纳，利于手术后病人的康复。笔者采取了同时经左胸第6或第7肋间前外侧切口进胸、经左腹直肌或腹直肌旁切口进腹，不切断左侧肋弓的经胸、经腹切口获得了满意的手术视野，方便了疝内容物的还纳，达到了手术中能详细探查胸腔、腹腔脏器的目的，保持了胸部的稳定性。术后病人无明显的心肺功能损害，取得了良好的手术治疗效果。Suffered from diaphragmatic hernia caused by injury or narrow winding, the patient is often in dangerous crisis. In order to create a better operation vision scope,re duce the possible damage to internal organs, privide convenient return of the hernia ma terial and reduce the occurrence of complications, it is necesary to improve on the previ ons chest only,abdomen only or chest and abdomen simultaneous incision. For this sake, the authors adopted the method of making simultneous incisions at the tip of the sixth or senth rib of the left chest and in muscles of the abdomen or left abdomen, with out breaking of any rib.The advantages of the new method arc that the operation vision scope is widened, detailed examination is rendered of the internal organs in chest and abdomen cavities, the goal is achieved to enable the convenient return of the hernia con tent and the chest frame is stabilized. The method is effective in curing the patient with out apparent damage to the heart or lungs.
