鼻内镜下射频热凝鼻后下神经和筛前神经及下鼻甲治疗常年性变应性鼻炎的远期疗效观察

摘　　要：目的探讨鼻内镜下射频热凝鼻后下神经、筛前神经治疗常年性变应性鼻炎(PAR)的疗效。方法将80例PAR患者随机分为两组各40例,观察组采用鼻内镜下射频热凝鼻后下神经、筛前神经及下鼻甲综合疗法,对照组采用鼻内镜下射频热凝蝶腭神经疗法,观察两种疗法的疗效和并发症,所有病例均随访2年。结果术后0.5、1、2年观察组的总有效率分别为100%、95%、82.5%;对照组分别为67.5%、47.5%、32.5%,两组各年期总有效率差异有统计学意义(P<0.01)。观察组无并发症发生,对照组术中有3例出血,术后9例出现眼干。结论鼻内镜下射频热凝鼻后下神经、筛前神经及下鼻甲治疗PAR具有疗效显著、操作简便、无副作用等优点,有良好的临床应用价值。Objective To study a new method for treating perennial allergic rhinifis （PAR）. Methods Eighty cases of PAR randomly divided into 2 groups. （1） In observation group,forty cases treated with nasal posterior-under nerve,ethmoidal nerve and infi-aturbinal radiofrequeney under nasal endoscope;（2） In control group,forty cases treated with sphenopalatine nerves radiofrequency under nasal endoscope. All the cases were follow-up two years postoperatlvely.Resuits In observation group,0.5,1,2 years total effective rate was 100% ,95% and 82.5% ,respectively while those of control group was 67.5% ,47.5% and 32. 5%. 0. 5,1,2 years total effective rate had significant difference between two groups （P 〈0. 01）. The complicatiom was not observed in observation group,massive intraoperation hemorrhage （n = 3 ） and xerophthalmia （n =9） was observed in control group. Conclusion The method that treating PAR with nasal posterior-under nerve,ethmoidal nerve and infraturbinal radiofrequency under nasal endoscope is highly effective,easy manipulation and few copmlications.
