摘 要:目的探讨 HBV 携带产妇血清、乳汁不同 HBV-DNA 含量母乳喂养的安全性,以指导母乳喂养。方法应用荧光定量 PCR(FQ-PCR)技术对93例 HBV 携带产妇血清、乳汁及婴儿血标本进行 HBV-DNA 定量检测。婴儿分别采用母乳和人工两种喂养方式。结果 68例血清 HBV-DNA 阳性产妇的乳汁中 HBV-DNA 阳性44例(64.71%),乳汁 HBV-DNA 阳性率随母血 HBV-DNA 载量的增加而增加,且2种标本 HBV-DNA 含量呈正相关(r=0.684,P<0.05);HBeAg 阳性和阴性产妇乳汁 HBV-DNA 阳性率差异有统计学意义(P<0.01);仅 HBsAg阳性产妇婴儿,母乳喂养组和人工喂养组均未发生 HBV 感染。结论血清 HBV-DNA 阳性产妇哺乳应结合乳汁HBV-DNA 检测;乳汁 HBV-DNA 检测对正确指导母乳喂养有一定的实用价值。[临床儿科杂志,2008,26(10):869-871]Objectives To quantitatively analyze hepatitis B virus (HBV)-DNA in serum and breast milk of HBV carrier postpartum women for information on safety of breast feeding. Methods HBV-DNA in blood, breast milk, and blood of their 24 month old infants from 93 HBV carrier postpartum women were detected by fluorescent quantitative PCR (FQ-PCR). Infants were breast-fed or bottle-fed. Results Forty-four eases of milk were detected HBV-DNA positive in sixty-eight serum HBV-DNA positive postpartum women (64.71%). The percentage of milk HBV- DNA positive was increased with higher blood HBV-DNA level in postpartum women. There was a positive relationship between milk and serum HBV-DNA levels (r = 0.684, P 〈 0.05). Milk HBV-DNA positive rate was significantly high- er in HBeAg positive than in HBeAg negative postpartum women (P 〈 0.01 ). No infants were infected by HBV in both breast feeding and bottle feeding .groups if their mothers are only HBsAg positive. Conclusions Milk HBV-DNA should be detected in serum HBV-DNA positive postpartum women. Milk HBV-DNA detection have practical values for guidance of properly breast feeding.
