老年患者胃镜检查时心脏自主神经功能的变化

摘　　要：目的观查和分析老年患者胃镜检查时心脏自主神经功能的变化及意义。方法按序选取30例胃镜检查患者,年龄60～84岁;采用心率血压监测仪测定患者胃镜检查前及检查时心率变异性(HRV)包括平均R-R间期(RRI)、总变异性(TV)、低频成份(LF)、高频成份(HF)和低频成份/高频成份(LF/HF)和压力反射敏感性(BRS)包括血压总变异(BRSTV)、血压谱密度的低频成份(BRSLF)、血压谱密度的高频成份(BRSHF)、血压谱密度的甚低频成份(BRSVLF)的变化,分析检查前及检查时患者自主神经功能的变化。结果检查时HRV指标RRI、TV、LF、HF和BRS指标BRSTV、BRSLF、BRSHF、BRSVLF均较检查前有明显升高(P=0.00),检查时LF/HF也较检查前升高(P=0.042)。结论老年患者胃镜检查时交感神经和迷走神经兴奋性均增强,交感神经兴奋性增强可能更明显。Objective To investigate effects of taking gastroscopy on the function of cardiac autonomic nerves in old patients. Methods Heart rate variability （HRV） and baroreflex sensitivity （BRS） were performed in thirty patients before and during taking gastroscopy, including average R-R interval （ RRI）, total variability （ TV）, low frequency （ LF）, high frequency （ HF）, LF/HF and BRS of TV, LF, HF and very low frequency （VLF） , such as BRSTV,BRSLF , BRSHF and BRSVLF. Results Indexes of HRV including RRI,TV,LF, HF and BRSTV, BRSLF, BRSHF and BRSVLF were significantly higher during taking gastroscopy than those before （ P = 0. 00 ） , LF/HF during taking gastroseopy was higher than that before （ P = 0. 042）. Conclusions Excitability of cardiac autonomic nerves including sympathetic nerve and pneumogastric nerve is enhanced during taking gastroscopy, especially sympathetic nerve.
