改良膀胱灌注法治疗膀胱癌术后患者的护理研究

摘　　要：目的：确保化疗药液保留在膀胱内足够的时间，减轻化疗药物引起的膀胱刺激症状，以减轻患者的痛苦和提高治疗效果。方法：随机将40例膀胱癌部分切除术后的患者分为改良组20例和传统组20例，改良组在膀胱灌注化疗药液后注入5ml空气、保留尿管、夹紧尿管下端连接引流袋，将药液保留2h后，放开夹子把药液引流至引流袋，最后拔除尿管；传统组采用传统的膀胱灌注法；观察并比较两组化疗药液保留在膀胱内的时间及膀胱刺激症状的情况，进行统计分析。结果：以药液能保留在膀胱内2h为标准，改良组达标率为94．3％，传统组达标率为61．3％，两组药液保留在膀胱内时间的比较有显著意义（P〈0．001）；改良组发生膀胱刺激症状的情况比传统组轻。结论：采用改良膀胱灌注法进行膀胱灌注治疗使药液按要求保留在膀胱内的时间得到保证，并能减轻患者的膀胱刺激症状，对提高治疗效果和生活质量都有重要意义。Objective ：To ensure adequate time of ehemotherapeutics remain in the urinary bladder, to lessen irritation sign of bladder and suffering, to elevation therapeutic efficacy. Methods ：40 patients with bladder cancer after operation were randomly divided into two groups to intravesieal instillation improved group were infused 5ml air after infused chemotherapeutics and remain urinary, canal,after 2 hours, to exelcymosis urinary canal. Tradition group use the method of orthodox irrigation of bladder. Results ： The standard is physic liquor can remain in the bladder about 2 hours, the improved group was 94. 3% , the tradition group was 61.3%. Significant difference existed between the two groups（ P 〈 0. 001 ） ;the irritation sign of bladder was lighter in the improved group. Conclusion ： It is enough time to ensure physic liquor can remain in the bladder through adopt improved method,and lessen irritation sign of bladder. It is important significant to increase therapeutic efficacy and improve quality of life.
