摘 要:目的:探讨动态三维超声心动图对先天性心脏病的诊断价值。方法:选取48例先天性心脏病患者分别行二维超声心动图及动态三维超声心动图检查,采集二维、动态三维图像,分析比较2种超声显像技术对心血管解剖结构的显示能力,并对24例室间隔缺损(VSD)、14例房间隔缺损(ASD)患者缺损最大直径的二维及三维超声测值与手术测值进行比较。结果:所有患者均获得满意的三维图像,能清晰显示心内膜、二尖瓣、三尖瓣及主动脉瓣,准确显示心血管三维空间解剖畸形;应用三维超声心动图测得LVEDV、LVESV、LVSV、EF值与二维超声的测值相比相关性良好(1值分别为0.83、0.87、0.85及0.80,P均〈0.01),2DE测量结果偏低,但两者比较差异无统计学意义(P〉0.05);三维超声实时显示室间隔或房间隔缺损的空间位置、形态、测量的缺损最大直径与手术测值无显著差异(P〉0.05)。结论:动态三维超声心动图能实时显示先天性心脏病的立体结构及功能状态,为外科手术提供更多信息。Objective: To investigate the clinical application of dynamic three - dimensional echocardiography ( 3 - DE) in patients with congenital heart diseases. Methods: Two - dimensional echocardiography ( 2 - DE) and 3 DE were performed in 48 patients with various congenital heart diseases, in which the results of 38 patients derived by echocardiography were compared with that of operations. Results: Compared with conventional 2 -DE technique, 3 -DE had the advantages in the following aspects:satisfactory 3DE imaging were acquired in all patients to dearly .display endocardinm, mitral valve,tricuspid valve, aortic valve and the cardiovascular three dimensional anatomic malformation. 3DE provided more abundant diagnosing information than 2 - DE. The LVEDV,LVESV,LVSV and LVEF from 3DE were showed excellent correlation with those measured with 2DE(r = 0. 83,0. 87,0. 85 and 0. 80, respectively, P 〈 0. 01 ). The results from 2DE were lower than that from 3DE but with no significant differences between them ( P 〉 0. 05 ). Displaying the location and morphology of atrial or ventricular septal defects instantly, without significant difference between the diameters of atrial or ventricular septal defects derived by 3 DE and surgical operation (P 〉 0. 05 ). Conclusion:The dynamic three - dimensional echocardiography provides a reliable techniques displaying cardiac structures and function unobtainable by 2 - DE, which may enhance the qualitative and quantitative diagnostic accuracy and produce potential clinical values for congenital heart diseases.
