原发性面神经瘤围手术期护理

摘　　要：目的探讨原发性面神经瘤围手术期护理要点。方法对5例原发性面神经瘤（其中面神经鞘膜瘤4例，面神经纤维瘤1例）的患者于术前做好心理护理及完善各项检查，调整患者的心理状态以尽早适应手术，术后加强伤口的观察及护理，注意面瘫的发生、发展及护理。结果5例患者中，3例经乳突径路，2例经FiSCh径路，切除肿瘤同时保留全部或部分面神经纤维3例，肿瘤与面神经同时切除2例，面神经与舌下神经吻合1例。经及时治疗与护理，术后病情恢复良好出院。随访结果所有病例肿瘤均未复发。3例保留面神经的患者1例面瘫完全恢复；2例获得Ⅰ～Ⅱ级恢复。1例神经吻合获Ⅲ级恢复。结论早期彻底切除肿瘤是面神经瘤唯一有效的治疗，而加强围手术期护理，使病人尽早接受手术，预防并及时处理并发症是提高手术成功率，保证疗效的重要环节。Objective To investigate the main points of nursing about the primary facial nerve tumor in the perioperative period. Methods There are five patients with the primary facial nerve tumor （4 patients with the facial nerve neurinoma, one with the facial neurofibrom）, 1In order to adjust the patients＇s mental state to adapt the operation, we do much work of the mental nursing and consummate correlative examination in the preoperative. After the operation, we reinforce the observation and nursing of the wound, meanwhile, and pay more attention to facial paralysis when it occur, or develop. Results Of the 5 patients, 3 patients with transmastoid approach, 2 patients with trans-Fisch approach, 3 patients with excising tumor and remaining all or part of facial nerve fiber, 2 patients with excising tumor and facial nerve fiber at the same time, 1 patient with anastomosing of the facial nerve and cranial nerve. With the treatment and nursing in time, all the patients recover well and discharge postoperative. All the patients don＇t find recurrence in the follow-up visit after discharge. Conclusions Excising tumor thoroughly in the nonage is the only one effective treatment, but to improve the achievement ratio and ensure the therapeutic effect, we reinfore the nursing of perioperative period, for example, to help the patients accept the operation as early as possible, and to prevent or dispose complication in time ,which is a very important link.
