摘  要:目的探讨川崎病(Kawasaki disease,KD)患儿急性期血清N端脑利钠肽原(N-terminal pro-brain natriuretic peptide,NT-pro BNP)的变化及其临床意义。方法测定25例确诊为KD的患儿急性期血清15例急性上呼吸道感染患者,血清NT-pro BNP、外周血白细胞计数(white blood cell,WBC)、C反应蛋白(C-reactive protein,CRP)、血细胞沉降率(Erythrocyte sedimentation rate,ESR),并常规超声心动图测定患者左室射血分数(Left ventricular ejection fraction,LVEF)及冠状动脉大小。结果KD患儿急性期血清NT-pro BNP水平为1 690.3(65.0~22 489.0)pg/ml,而对照组小儿血浆中NT-pro BNP水平为88.7(10.9~200.0)pg/ml,两组相比差异具有统计学意义(P<0.05)。KD患儿急性期血浆NT-pro BNP水平与患儿急性期CRP水平呈正相关(r=0.60,P<0.05),与LVEF与无相关(r=-1.99,P>0.05)。结论血清NT-pro BNP水平在KD急性期显著升高,有助于KD的诊断。Objective To evaluate the change and clinical value of serum N - terminal pro - brain natriuretic peptide (NT -pro BNP) in the acute stage of Kawasaki disease(KD) in children. Methods Twenty five patients with KD were enrolled and fifteen patients with acute upperrespiratory infection were studied as controls. Blood sample was obtained to measure serum NT - pro BNP concentrations in the acute stage of KD and in the acute phase of the control patients. Serum NT - pro BNP was measured using electrochemiluminescence immunoassay. White blood cell counts ( WBC ) , C - reactive protein (CRP) and Erythrocyte sedimentation rate(ESR) were also measured in acute phase in both groups. All patients with KD had complete echocardiographic study to measure left ventricular ejection fraction (LVEF) and to check coronary lesions. Results The median serum NT - pro BNP concentration in patients with KD in the acute phase was1690.3 (65.0 - 22489.0 ) pg/ml, and it was 88.7 ( 10.9 - 200.0 ) pg/ml in patients of control group. The serum NT - pro BNP in patients with KD in the acute phase was significantly higher than that of the control group ( P 〈 0.05 ). Through linear regression analysis, in acute phase of KD, the NT - pro BNP level correlated positively with CRP ( r = 0. 60, P 〈 0. 05) , however, there was no significant correlation between the serum concentrations of NT - pro BNP and LVEF( r = - 1.99, P 〉 0.05). Conclusion The serum NT - pro BNP concentration increased in the acute phase of KD, which might be beneficial to diagnosis of KD.
