下肢淋巴水肿的整复治疗临床研究

摘　　要：目的采用不同的手术方法治疗肢体淋巴水肿患者17例,以研究和探索淋巴水肿的修复效果。方法用脂肪抽吸的方法治疗肢体淋巴水肿6例,采用皮瓣下软组织清除、多余皮肤组织切除法治疗4例;肢体淋巴组织增生肿块切除加自体大张皮肤回植7例。结果1例脂肪抽后效果不理想,1例淋巴水肿增生组织切除后回植皮肤愈合欠佳,其余15例效果满意。结论淋巴水肿Ⅱ期选用脂肪抽吸;Ⅲ期采用皮瓣下修复,Ⅳ期选用增生组织切除自体皮肤回植方法较好。Objective To investigate the restorative results of extremity lymphoedema treated with different surgical procedures. Methods Three treatment methods were used in 17 cases including liposuction （6 cases）, removing the soft tissue under the skin flap （4 cases）, lymphoproliferative tumor resection combined with large piece of skin autografting for repair （7 eases）. Results Apart from 1 case of not performing well after liposuction, 1 case of skin healing poor after lmphoproliferative tumor resection plus large piece of skin autografting for repair, and the rest cases satisfied with the results. Conclusion It is a better choice for the treatment of limb lymphedema that liposuction on Phase Ⅱ lymphedema, repair under the skin flap on Phase Ⅲ and hyperplasia excision combined with skin autografting on Phase IV.
