摘　　要：[目的]观察硬膜外自控镇痛(PCEA)对妊娠高血压综合征病人剖宫产术后康复的影响。[方法]将120例妊娠高血压综合征剖宫产病人随机分为观察组(术后PCEA镇痛)和对照组,每组各60例,两组均采用硬膜外麻醉。观察组采用PCEA镇痛,镇痛时间为48h。对照组术毕拔出硬膜外导管,疼痛时肌肉注射盐酸哌替啶,每次50mg。对两组病人术后24h镇痛效果、24h阴道流血量、肛门排气时间、初乳分泌时间及术后24h、48h血压进行比较。[结果]观察组镇痛效果明显优于对照组;术后24h阴道出血量、肛门排气时间及泌乳时间与对照组比较差异无统计学意义;术后24h、48h血压比对照组低。[结论]对妊娠高血压综合征病人剖宫产术后采用PCEA镇痛,是一种比较理想的镇痛方法,有利于病人术后康复。Objective： To observe the influence of epidural self-control anal- gesia （PCEA） on postoperative rehabilitation of patients with pregnancy - induced hypertension syndrome （PIH） undergoing cesarean section. Methods： A total of 120 PIH patients undergoing cesarean section were divided into test group （postoperative PCEA） and control group （60 cases each）. Epidural analgesia was carried out for all the two group cases. PCEA had been adopted for test group cases for 48 hours. The epidural catheter was extracted at the end of operation in the control group cases and intramuscular injection of pethidine hydrochloride for their pain at the dose of 50 mg. The analgesia effect and the amount of vaginal bleeding on 24h postoperative, the time of passage of gas by anus, secreting time of colostral milk and hypertension at 24h and 48h after operation were compared between both groups. Results： The analgesia effect of test group cases was significantly better than that of control group. There was no statistical significance in terms of the amount of vaginal bleeding on 24h after operation, time of passage of gas by anus and secreting time of colostral milk between both groups. Blood pressure at 24h and 48h after operation in test group cases wasLower than that of control group cases. Conclusion： Carrying out PCEA for patients with pregnancy - induced hypertension syndrome （PIH） after undergoing cesarean section is an ideal analgesia method. And it is helpful for postoperative rehabilitation of patients as well.
