急性百草枯中毒患者血清载脂蛋白A1的变化及临床意义

摘　　要：目的:探讨急性百草枯中毒(APP)患者血清载脂蛋白A1(APOA1)的变化及意义。方法:急性百草枯中毒患者共41例,根据患者入院时服毒量及临床体征分为轻度中毒组及中、重度中毒组,于第2天采集空腹静脉血3ml,以免疫比浊法检测血清APOA1,并随机选择28例门诊健康体检者作为对照组。结果:APP患者血清APOA1水平[(1.226±0.306)g/L]较对照组[(1.392±0.125)g/L]比较显著降低(P<0.05);且中、重度中毒组降低率(66.7%)较轻度中毒组(34.6%)明显(P<0.01)。结论:APP时APOA1水平显著降低,参与了脏器损害的发生,二者互为因果,加重了APP全身炎症反应综合征(SIRS)及脏器损伤的发生,同时APOA1的检测对了解APP患者机体的损害状况、指导临床治疗有一定的帮助。Objective： To investigate the changes and clinical significances of serum apolipoprotein A1 （APO A1） of patients with acute parquat poisoning （APP）. Methods： 41 patients were divided into two groups： the mild poisoning group and moderate and severe group according to the amount of taking poison and clinical physical signs on admission. 3 ml fasting venous blood was obtained from the above patients, and then immune turbidimetry was used to determine the serum apolipoptotein A1 on the second day. 28 health subjects with medical check-up of outpatients were randomly selected as the control group. Results： The level of serum apolipoptotein A1 of patients with APP [（1.226±0.306） g/L] was significantly lower than that in control group[（1.392±0.125） g/L, P〈0.05]; the decreasing rate of moderate and severe group （66.7%） was considerably higher than that in mild group（34.6%） （P〈0.01）. Conclusion： The level of serum apolipoptotein A1 of patients with APP is significantly decreased. APO A1 takes part in the occurrence organ damage. There is a reciprocal causation between APO A1 and patients with APP. APO A1 aggravates the occurrence of systemic inflammatory response syndrome （SIRS） and organ damage. Meanwhile, the detection of APO A1 contribute to understanding the physical status of patients with APP as well as guiding the clinical medication.
