摘 要:目的探讨人重组粒细胞集落刺激因子(rhG-CSF)预激的青春期儿童和成人健康供者骨髓采集物造血和免疫学特性的异同。方法标本来自2004年1月至2007年9月在北京大学人民医院接受allo-HSCT的162例亲缘健康供者,其中青春期儿童23例,对其体内应用rhG-CSF5μg/(kg·d)连续4~5d,第4天和第5天分别采集骨髓和外周血采集物,用流式细胞仪测定骨髓采集物中的淋巴细胞、CD3+细胞、CD4+细胞、CD8+细胞、CD1+4细胞、CD3+4细胞以及CD3+CD4-CD8-调节性T细胞的数量,并比较了青春期儿童和139例成人供者(分2组:A组70例,年龄≤38岁;B组69例,年龄>38岁)rhG-CSF预激骨髓(G-BM)的造血和免疫学特性。结果23例青春期儿童供者每微升G-BM中的淋巴细胞2472(1346~5037)、CD3+细胞1678(758~3967)、CD8+细胞734(329~1602)以及CD3+4细胞74(26~237),含量显著高于B组供者[含量分别为1927(506~4757)、1225(325~3682)、460(113~2210)]和49(11~220)(P<0.05),与A组供者[2208(591~6618)、1393(307~4791)、636(112~2575)和76(22~505)]差异无统计学意义(P>0.05);青春期儿童供者G-BM中CD4+细胞/CD8+细胞的比值0.95(0.55~2.73)小于B组供者1.24(0.41~2.87)(P<0.05),与A组1.03(0.49~2.11)的差异无统计学意义(P>0.05)。青春期儿童供者每微升G-BM中所含CD14+细胞1091(521~2674)、CD3+CD4-CD8-调节性T细胞136(48~487)的数量以及CD3+CD4-CD8-调节性T细胞/CD3+细胞的比值0.09(0.04~0.30)都显著高于A组998(371~2958)、96(11~249)、0.06(0.01~0.21)和B组894(265~1997)、65(9~390)、0.05(0.01~0.18)成人供者(P<0.05)。结论rhG-CSF预激的青春期儿童供者骨髓采集物中含有的造血干/祖细胞数量高于年龄>38岁的成人供者,而与年龄≤38岁的数量相当;青春期儿童供者G-BM在免疫组成方面优于成人供者。Objective To compare the hematopoietic and immunological characteristics of granulocyte colony-stimulating factor primed bone marrow grafts ( G-BM ) between pediatric and adult donors. Methods Twenty-three pediatric donors and 139 adult donors were treated with rhG-CSF [ 5 μg/( kg ·d) ] injected subcutaneously for five consecutive days. The adult donors were classified into groups A (donor age ≤ 38 years ) and B ( donor age 〉 38 years) according to median age,70 and 69 donors for group A and B,respectively. Bone marrow grafts and peripheral blood stem cell grafts were harvested on day 4 and day 5. The quantities of lymphocytes, CD3^+, CD3^+CD4^+, CD3^+ CD8^+, CD14^+, CD34^+ cells and CD3^+ CD4^-CD8^- regulatory T cells were determined by multi-color flow cytometry. The hematopoietic and immunological characteristics of G-BM between pediatric and adult donors were compared. Results The counts of lymphocytes 2472 ( 1346 - 5037 ), CD3^+ 1678 ( 758 - 3967 ), CD3^+ CD8^+ 734 ( 329 - 1602 ), and CD43^+ 74 ( 26 - 237 ) cells per microliter in G-BM of pediatric donors were significantly higher than those in group B 1927 ( 506 - 4757 ) , 1225 ( 325- 3682),460(113 -2210),and 49(11 -220) (P 〈 0. 05 ) and similar to those 2208 ( 591 - 6618 ) , 1393 ( 307 - 4791 ) ,636 ( 112 - 25751, and 76 ( 22 - 505 ) ( P 〉 0.05 ). The ratio of CD4^+ cells/CD8^+ cells 0. 95 (0. 55 - 2. 73 ) in G-BM of pediatric donors was significantly lower than that in group B 1.24 ( 0. 41 - 2. 87 ) ( P 〈 0.05 ) and similar to that in group A 0. 69 ( 0. 20 - 2. 23 ) ( P 〉 0.05 ). The quantities of CD14^+ cells 1091 ( 521 - 2674 ), and CD3^+ CD4^- CD8^- regulatory T cells 136(48 -487) per microliter and the ratio of CD3^+ CD4^-CD8^- regulatory T cells/CD3^+ cells 0. 09(0. 04 -0. 30 ) in G-BM of pediatric donors were significantly higher than those of adult donors, including groups A 998 (371 - 2958),96�
