围术期预防性使用抗菌药物的回顾性分析

摘　　要：目的对围术期预防性应用抗菌药物进行回顾性分析，促进围术期合理应用抗菌药物。方法调查围术期中没有明显感染征象的手术病人的性别、年龄、手术日期、手术名称、切口类别、术前是否将抗生素带入手术室、用药天数、使用药物种数等资料，输入数据库进行统计分析。结果自2007年1月至12月共入选1148例，术前抗生素未带入手术室仅44例，占3．8％。不同类别切口使用抗生素种类比较：未用或仅用1种抗菌药占78．0％，二联用药占17.3％，三联及以上用药占4．6％。不同类别切口使用抗生素天数比较：抗生素使用超过3d占43．7％；抗生素使用超过7d占10．8％；抗生素使用超过10d占5．2％。没有联合用药且用药天数≤3d占50．0％。结论提示合理使用抗菌药物的理念开始深入到每个医护人员的心中，滥用和过度使用抗菌药物的现象有所遏制；但是某些科室或某些医生滥用和过度使用抗菌药物的现象仍然存在。Objective Carry on the review analysis of preventive application of antibiotics in perioperative period, promote reasonable application of antibiotics in perioperative period. Methods Investigate surgery patients without obvious infection sign in perioperative period about their sex, age, surgery date, surgery name, the margin category, whether taking antibiotics to the operating room before operation, the day number of medication , kind of medicine used, data inputted and carry on the statistical analysis. Results 1 148 examples are selected from January, 2007 to December, only 44 examples that the antibiotic has not been taken to the operating room before the operation, account for 3.8%. Different category margin use antibiotic type comparison： Not with or only use 1 kind of antibiotics account for 78.0%, bigeminy accounts for 17.3%, the trigeminy and above account for 4.6%. Different category margin use antibiotic days comparison： The antibiotic use surpassing 3d accounts for 43.7%; The antibiotic use surpassing 7d accounts for 10.8%; The antibiotic use surpassing 10d accounts for 5.2%. No united medication and the medication days less than 3d account for 50.0%. Conclusions idea of prompting reasonable use antibiotics starts to penetrate into the heart of each medical care personnel, the phenomenon of abuseing antibiotics have been prevented, but it still exists in certain departments or doctors.
