摘 要:目的通过荚膜组织胞浆菌素纯蛋白衍生物(P-HTPM)对荚膜组织胞浆菌(HC)和不同分枝杆菌致敏豚鼠的皮肤试验,检测P-HTPM用于组织胞浆菌病(HP)和结核病鉴别诊断的特异性和敏感性。方法用结核分枝杆菌(MTB)和22种非结核分枝杆菌以及荚膜组织胞浆菌(HC)分别致敏豚鼠,经皮内注射结核杆菌纯蛋白衍生物(TB-PPD)、胞内分枝杆菌纯蛋白衍生物(PPD-B)、P-HTPM或国外同类产品HDS,检测P-HTPM的特异性;将P-HTPM原液分别稀释成1、2、4、8μg/ml,与HDS同时以轮圈法注射于经HC致敏的豚鼠,检测P-HTPM的敏感性;用MTB、MTB+HC和不同浓度的HC分别致敏豚鼠,再分别经皮内注射TB-PPD和P-HTPM,检测交叉反应性。结果P-HTPM与HDS对HC致敏的豚鼠均呈阳性反应,而对结核及其他分枝杆菌致敏的豚鼠均呈阴性反应;不同浓度的P-HTPM均可诱导HC致敏豚鼠的皮肤反应,并随P-HTPM浓度升高,皮肤反应强度略有增强,1μg/ml的P-HTPM即与HDS诱导的皮肤反应等效;TB-PPD及P-HTPM对自身抗原致敏的豚鼠均可诱导较强的皮肤反应,且明显强于MTB+HC混合抗原致敏豚鼠诱导的皮肤反应;TB-PPD对HC致敏的豚鼠个别呈弱阳性,但随HC致敏浓度的升高,其阳性率下降。结论P-HTPM用于HP和结核病的鉴别诊断具有较好的特异性和敏感性。Objective To determine the specificity and sensitivity of purified protein derivative of histoplasmin (P-HTPM) in differential diagnosis of histoplasmasis(HP) and tuberculosis by skin test in guinea pigs sensitized with Histoplasma capsulatum and various mycobacteria. Methods Sensitize guinea pigs with Mycobacterium tuberculosis(MTB) and 22 kinds of other mycobacteria as well as Histoplasma capsulatum(HC) respectively. The above-mentioned sensitized guinea pigs were injected i. d. with purified protein derivative of tubercle bacillus (TB-PPD), purified protein derivative of Mycobacterium intracellulare (PPD-B), P-HTPM and imported histoplasmin HDS to test the specificity of P-HTPM. The HC-sensitized guinea pigs were simultaneously injected with P-HTPM, diluted to concentrations of 1, 2, 4 and 8 μg / ml, and HDS respectively, to test the sensitivity of P-HTPM. Guinea pigs were sensitized with MTB, MTB+HC and various concentrations of HC respectively, then injected i. d. with TB-PPD and P-HTPM to test the cross reactivity. Results Both the results of skin tests on P-HTPM and HDS in HC-sensitized guinea pigs were positive, and those in the guinea pigs sensitized with MTB and other mycobacteria were negative. The P-HTPM at various concentrations induced skin reactions in HC-sensitized guinea pigs, and the reaction strength increased with the increasing P-HTPM concentration. The skin reaction strength induced by 1 μg / ml of P-HTPM was equivalent to that by HDS. Both TB-PPD and P-HTPM induced stronger skin reactions in the guinea pigs sensitized by the self-antigens than those by MTB+HC. TB-PPD induced weak positive reaction in individual guinea pigs sensitized by HC. However, the positive reaction rate decreased with the increasing HC concentration. Conclusion P-HTPM showed high specificity and sensitivity in differential diagnosis of HP and tuberculosis.
