药学监护与ADR监测为一体工作模式的实践

摘　　要：目的:为医院开展药物不良反应药学监护提供理论依据与切实可行的技术手段。方法:利用医院信息化平台实现临床科室自动上报ADR;临床药师查房、会诊,直接面对病人,参与ADR患者靶药的判断,治疗方案的调整,全程监控ADR转归。结果:截止2007年12月,临床药师全程监护、参与救治ADR患者800多例,临床科室主动上报ADR例数明显增多;能即时应对严重、突发、群发ADE。结论:药学监护与ADR监测为一体的工作模式,有利于临床药师开展药学服务,促进合理用药。Objective：To develop a practical technology method and theoretical basis for the implementation of adverse drug reaction monitoring and pharmaceutical care in hospital.Methods：On the basis of informationized platform of our hospital,clinical departments automatically reported ADR,pharmacists participated in the clinical rounds,consultation with doctors,judgement of the target drugs for patients with adverse drug reactions,regulation of the therapeutic strategies and monitoring the turnover of the adverse drug reactions.Results：The clinical pharmacist participated in the treatment and guardianship of about 800 ADR cases,and the number of the automatically reported ADR by clinical division increased until Dec in 2007.Conclusion：The work pattern integrating pharmaceutical care and adverse drug reaction monitoring is of great benefit to carrying out the pharmaceutical sevice from clinical pharmacist and promoting rational drug use.
