微创胸腔闭式引流术治疗自发性气胸

摘　　要：目的:研究微创胸腔闭式引流术治疗自发性气胸的临床疗效及其并发症。方法:将38例自发性气胸患者,随机分为2组,研究组采用微创胸腔闭式引流术治疗,对照组采用传统的胸腔闭式引流术治疗。结果:研究组19例中肺完全复张18例,无效1例,有效率94.74%,高于对照组;研究组肺复张时间(2.14±1.92d)、术后胸痛时间(1.75±1.12d)、拔管后伤口愈合时间(2.25±0.72d)、住院天数(7.25±2.81d)均比对照组短,术后并发症少,两组有显著差异。结论:采用微创胸腔闭式引流术治疗自发性气胸的操作简单方便,组织损伤小,患者易于接受,且其疗效好,术后并发症少,拔管后伤口愈合快,住院时间明显缩短。Objective：To study the curative effects and complications in treating spontaneous pneumothorax through thoracic closed drainage by using thin silica gel catheter. Methods：Thirty-eight cases of spontaneous pneumothorax were divided into 2 groups （n = 28） randomly： one had the thoracic closed drainage through thin silica gel catheter, the other had orthodox method. Their therapeutic effects and complications were investigated. Results： In research group the success of alveolar recruitment was obtained in 18 out of 19 and only one case was ineffective. The total effective rate（94.74% ） in research group was higher than that in control group. As compared with those in control group, the average time of alveolar recruitment （2.14 ± 1.92 d） and the time of chest pain after operation （1.75 ± 1.12 d） were less in research group, and the wound healing time after the re- moval of catheter （2.25 ± 0.72 d） and the average duration hospitalization （7.25 ± 2.81d） were also less in the research group. The research group showed low complication rate in treating spontaneous pneumothorax and there were significant difference between the two groups. Conclusion：The treatment of spontaneous pneumothorax through thoracic closed drainage by using thin silica gel catheter is a satisfactory remedy with less side effect, fast healing, simple , more comfortable, fewer complications shorten hospitalization.
