摘 要:目的:观察针刀松解法对实验性骨关节炎(OA)兔血清基质金属蛋白酶3和13(MMP-3、MMP-13)的影响,探讨其防治OA的机制。方法:正常新西兰兔7只为正常组,另27只造模成功兔按随机数字表法随机分为模型组(n=7)、针刀组(n=10)和电针组(n=10),用关节制动方法复制出OA模型。针刀组于关节周围找到条索状物并定点,以纵行疏通法为主兼以横行剥离法行针刀松解术,每周1次,共治疗3次;电针组取“阴陵泉”“阳陵泉”“梁丘”穴,电针频率2~100Hz,强度2mA,每次电针10min,每周治疗3次,共3周。参照Lequesne膝关节临床评估级别中的关节活动范围进行评估;用ELISA方法检测外周血清MMP-3、MMP-13含量。结果:造模后患肢膝关节活动范围明显减小(P〈0.05);针刀组和电针组经治疗后患肢膝关节活动范围明显增大(P〈0.05)。模型组MMP-3、MMP-13水平明显升高,与其它组相比差异有统计学意义(P〈0.05);针刀组MMP-3、MMP-13水平均降低,与模型组比较差异有统计学意义(P〈0.05),但与电针组相比差异无统计学意义(P〉0.05)。结论:针刀松解治疗OA兔,能够使外周血清MMP-3和MMP-13水平下降,从而有可能使软骨退行性变的部分因素得到控制,使关节软骨变性减轻,起到治疗作用。Objective To observe the effect of acupotomy lysis on serum matrix metalloproteinase (MMP)-3 and MMP- 13 contents in osteoarthritis (CA) rabbits. Methods A total of 34 New Zealand rabbits were randomly divided into normal group in =7), model group in =7), acupotomy group in = 10) and electroacupuncture (EA) group in = 10). CA model was established by left-hindlimb-joint-immobilization method. Acupotomy lysis was applied to the affected joint, once a week for 3 weeks. EA (2/100 Hz, 2 mA) was applied to "Yinlingquan" (SP 9), "Yanglingquan" (GB 34) and "Liangqiu" (ST 34) on the left hindlimb for 10 min, 3 times a week for 3 weeks. Serum MMP-3 and MMP-13 contents were detected with enzyme linked immunosorbent assay (ELISA) method. Results In comparison with model group igrade Ⅲ : 1 case; grade Ⅳ : 4 cases), the left kneejoint-motion range of rabbits in acupotomy and EA groups igrade Ⅰ : 1 and 1 case; grade Ⅱ : 4 and 3 cases; grade Ⅲ : 2 and 2 cases; grade Ⅳ : 1 and 2 cases separately) increased significantly (P〈0.05). Compared with normal group, serum MMP-3 and MMP-13 contents in model group increased significantly i P〈0.05) ; while compared with model group, both MMP-3 and MMP-13 levels of acupotomy and EA groups reduced considerably (P〈0.05). No obvious difference was found between acupotomy and EA groups in the left knee-joint motion range, and serum MMP-3 and MMP-13 levels (P〉0.05). Conclusion Acupotomy is effective in lowering serum MMP-3 and MMP-13 levels, and in improving CA rabbits' knee-joint motion range.
