摘 要:目的监测临床病原菌的耐药性变迁。方法1998年1月-2005年12月临床送检标本中分离的病原菌及药敏结果,统计学分析采用WHONET5软件。结果8年间共检出病原菌19 832株,产超广谱β-内酰胺酶(ESBLs)革兰阴性杆菌和耐甲氧西林葡萄球菌(MRS)呈逐年上升趋势,1998年产ESBLs大肠埃希菌、产ESBLs肺炎克雷伯菌、耐甲氧西林金黄色葡萄球菌(MRSA)和耐甲氧西林凝固酶阴性葡萄球菌(MRCNS)分别为17.0%、16.0%、63.4%和46.7%;2005年上升到51.4%、49.5%、85.6%、73.3%;各种菌株的检出比例和耐药性也在不断变化;总的趋势是葡萄球菌属的排序在上升,非发酵菌与发酵菌的排序也在交替上升。结论医院病原菌的耐药性变迁符合全球病原菌耐药性总趋势,但也有其特殊性,继续抓好病原菌耐药性监测,特别是ESBLs、MRS、VISA和VRSA的监测尤为重要。OBJECTIVE To survey the drug resistance variation of pathogens. METHODS We analyzed all of the pathogens isolated from Jan 1998 to Dec 2005 and the drug resistance results. WHONET5 software was used to analyze the data. RESULTS A total of 19 832 strains of pathogens were detected for eight years. There was ascending tendency in ESBLs and MRS. The ratio of strains and resistance also varied. The general trend was that the order of Staphylococcus was going up and the order of non-fermented and fermented bacteria was ascending alternately. CONCLUSIONS The resistance variation of pathogens in our hospital consists with the global tendency of bacteria resistance. Of course it has its own characteristics. The resistance surveillance should be continued especially for ESBLs, MRS, VISA and VRSA.
