摘 要:目的探讨简化调强放疗(sIMRT)的剂量验证方法是否可采用与三维适形放疗(3DCRT)照射技术相同的剂最验证方法。方法从接受sIMRT的1988例患者中随机抽取12例,用Pinnacle计划系统对每个患者分别做3DCRT、sIMRT、IMRT治疗计划,用MapCHECK Model 11752D半导体剂量仪,在医科达Precise直线加速器上作剂量验证测量。结果sIMRT剂量偏差为2%(2mm)、3%(3mm)的总平均通过率分别为90.5%、94.8%,比3DCRT的92.4%和95.8%分别低1.9%(t=2.19,P=0.040)和似乎低1.0%,但后者差异无统计学意义(t=1.52,P=0.144);剂量偏差为4%(4mm)的通过率为98.2%,比3DCRT的98.4%似乎低了0.2%,但差异也无统计学意义(t=0.50,P=0.623)。而sIMRT剂最偏差为2%(2mm)、3%(3mm)、4%(4mm)的通过率与IMRT的88.4%、93.3%、96.7%相比分别提高了2.1%(t=2.17,P=0.041)、1.5%(t=2.62,P=0.016)、1.5%(t=3.68,P=0.001)。结论sIMRT技术町简化IMRT治疗前剂量验证,在sIMRT技术试用期应对每例sIMRT计划进行类似IMRT的剂量学验证方法,以后可采用与3DCRT技术相同或相近的剂量验证方法.Objective To study if the dose verification technique for three dimensional conformal radiation therapy(3DCRT) can be applied for simplified intensity modulation radiation therapy (sIMRT). Methods From 1988 patients treated by sIMRT in our department,12 were chosen randomly for the study. For each case,3 different plans of 3DCRT, sIMRT, and IMRT were worked out with Pinnacle TPS, and the dose verification for each plan was carried out with Elekta Precise LA by using 2D diode-matrix of Map- CHECK Model 1175. Results For sIMRT, the overall average percentages of pass points for DD (DTA) 2% (2 mm) ,3% (3 mm)and 4% (4 mm)were 90.5% ,94.8% and 98.2% respectively,which were slightly worse when comparing with those of 3DCRT with deterioration of l. 9% ( t = 2.19, P = 0. 040), 1.0% ( t = 1.52,P = 0. 144) and O. 2% (t = 0.05,P = 0.623 ), but slightly better comparing with those of IMRT with increment of 2.1% (t =2.17,P =0. 041) ,1.5% (t =2.62,P =0.016) and 1.5% (t = 3.68,P =0. 001) for 2% (2 mm) ,3% (3 ram) and 4% (4 mm) ,respectively. Conclusions The sIMRT technique simplifies the complicated dose verification procedure of IMRT. When the sIMRT technique is formally used, the procedure of dose distribution verification for 3DCRT can be used directly for sIMRT.
