代谢当量及呼吸睡眠监测联合观察在中度阻塞性睡眠呼吸暂停低通气综合征疗效评估中的应用

摘　　要：目的:探讨PSG及代谢当量(METs)联合观察在中度阻塞性睡眠呼吸暂停低通气综合征(OSAHS)疗效评估中的应用。方法:66例中度OSAHS患者按就诊时间先后顺序,单数行UPPP加舌根射频消融术治疗33例,双数行舌托治疗33例。2种治疗方法分别行治疗前与治疗后1年同观察参数(AHI、最低血氧饱和度、代谢当量)配对t检验,分别对比观察2种治疗方法间同观察参数间的变化。结果:33例患者经UPPP加舌根射频消融术治疗后1年总有效率达86.7%(27/33);舌托治疗后1年总有效率达84.8%(28/33)。2种治疗方法同观察参数治疗前后分别行配对t检验,其变化均差异有统计学意义(均P<0.05);2种治疗方法治疗后行同观察参数配对t检验,其变化均差异无统计学意义(均P>0.05)。结论:PSG和METs联合观察可作为中度OSAHS患者的疗效评价参数组合之一;UPPP加舌根射频消融术与舌托治疗均为治疗中度OSAHS患者的有效方法,且疗效相近。Objetive：To detemine the value of treadmill exeeercise test in predicating patients with moderate OSAHS treated by uvulopalatopharyngoplasty（UPPP） and tone base reduction or oral appliance.Method：Sixty-six patients were treated by UPPP and tone base reduction or oral appliance.The data including PSG（AHI,SaO2） and treadmile exercice test（METs）was ananysised.Result：The success rate（pencentage of patients with at least 50% reducuction in AHI）of UPPP and tone base reduction was 86.7%,and 84.8% in oral appliance.The diffference（AHI,SaO2,METs or the success reat）between the groups of postoperation or oral appliance was nonsignificant（P〉0.05）.Conclusion：As to moderate OSAHS,the UPPP and tone base reduction or oral appliance may be one of the effective thrarapy.The data including PSG（AHI,SaO2） and METs may be very useful in predicating effectiveness of treatment to OSAHS.
