莫西沙星治疗老年社区获得性肺炎的临床研究

摘　　要：目的研究莫西沙星治疗老年社区获得性肺炎(CAP)的疗效、安全性。方法91例老年(≥65岁)CAP患者随机分为3组。A组(对照组)予莫西沙星静脉滴注,疗程10d;B组予莫西沙星静脉滴注,疗程共5d;C组予莫西沙星序贯治疗,疗程共5d。结果①A组、B组和C组痊愈率分别为68.8%(22/32)、66.7%(20/30)和62.1%(18/29),临床有效率分别为90.6%(29/32)、90.0%(27/30)和82.8%(24/29),细菌清除率分别为84.6%(11/13)、85.7%(12/14)和76.9%(10/13);这三个指标中,A、B、C三组间差异均无统计学意义。②A、B、C组不良反应发生率分别为9.4%(3/31)、6.7%(2/30)和6.9%(2/29),差异无统计学意义。结论莫西沙星短程序贯治疗老年CAP效果、安全性与莫西沙星短程、传统疗程静脉治疗相似。Objective To evaluate the efficacy and safety of the short-course and sequential moxifloxacin therapy in the treatment of community-acquired pneumonia （CAP） in elderly patients. Methods This analysis was based on the outcomes in 91 patients aged ≥ 65 years from a randomized, double-blind, controlled trial conducted at our hospital. Patients （ mild to moderate） were randomized to receive moxifloxacin therapy for 10 days or 5 days and sequential moxifloxacin therapy for 5 days. Results ① The overall cure rates and efficacy rates were 68. 8% and 90. 6% in the group A （ the control group） ,66.7% and 90. 0% in the group B,62. 1% and 82. 8% in the group C, respectively. The bacterial clearance rates were 84. 6 % in the group A, 85.7 % in the group B,76. 9 % in the group C, respectively. There were no significant differences （ P 〉 0. 05 ） among the three groups in the above results. ② The incidence of adverse drug reactions of the three groups were 9. 4% ,6. 7% and 6. 9 % ,respectively. There were no significant differences （ P 〉 0. 05 ） among the three groups in the above results. Conclusion This analysis finds that moxifloxacin sequential therapy for 5 days is well tolerated in the treatment of CAP in elderly patients. Clinical success rates are statistically similar between moxifloxacin sequential therapy for 5 days and moxifloxacin therapy for 5 or 10 days.
