摘  要:目的:观察川芎嗪注射液治疗2型糖尿病(T2DM)合并周围神经病变(DPN)的临床疗效及可能机制。方法:2003年6月-2006年6月期间T2DM合并DPN病人112例,在相同的降糖治疗的基础上,随机分为治疗组56例,以川芎嗪注射液治疗;对照组56例,以维脑路通注射液治疗;疗程均为2周,观察两组空腹血糖(FPG)、餐后2h血糖(PPG2h)、糖化血红蛋白(HbA1C)、超敏C反应蛋白(hsC-RP)、尿微量白蛋白(UMA)、内皮素(ET)、血栓素B2(TXB2)、6-酮-前列环素F1α(6-Keto-PGF1α)治疗前后的变化。结果:川芎嗪注射液治疗组显效率、总有效率分别为69.64%、96.43%,与对照组(41.07%、67.86%)比较有非常显著性差异(P<0.01);治疗组治疗后与对照组比较,胫前神经、腓神经传导速度明显改善(P<0.01);FBG、PPG2h、HbA1c略下降(P>0.05),但无统计学意义;6-Keto-PGF1α明显升高(P<0.05),hsC-RP、UMA、ET、TXB2明显下降(P<0.05)均有明确的统计学意义。结论:川芎嗪能够更有效减轻DPN患者的临床症状,其机制可能是通过改善患者血管内皮细胞功能达到治疗目的。Objective:To observe the clinical effect of lignstrazine injection to treat diabetic peripheral neuropathy (DPN), and to explore its mechanism of action. Methods:This study was carried out on 112 patients with DPN during Jun. 2003 to Jun. 2006. On the base of the same treatment for Type 2 diabetic mellitus,the patients were randomly divided into two groups which included the experimental group and the control group. The experimenta group had 56 patients who were treated with lignstrazine injection while the control group which had 56 patients was treated with venoruton injection. We osberved the changes before and after the treatment in the two groups during the treatment period of two weeks. The FBG, PPG2h, HbA1c, hsC - RP, UMA, 6 - Keto - PGF1α, ET and TXB2 in both groups were observed. Results : The effects of ligustrazine injection was significantly better than venoruton injection( P 〈 0.01 ). The effective rate and total effective rate in treatment group were 61.25% and 82.50% respectively, those in control group were 20.00% and 45.00% ,there was a significant difference between two groups ( P 〈 0.01 ) ; after treatment the conduction velocity of pretibial nerve and peroneal nerve in treatment group was significantly improved, as compared with that of control group (P 〈0.01 );FBG, PFG and HbA1c were slightly decreased (P 〉 0.05 );6 -Keto- PGF1α was obviously increased( P 〈 0. 05) ; hsC - RP, UMA, ET and TXB2 were significantly decreased( P 〈0. 05). Conclusions : Lignstrazine injection can relieve the clinical symptoms of the patients with DPN more effectively, and its mechanism may berelated to the improvement of the function of vascular endothelial cells.
