摘  要:目的探讨脉压对老年高血压合并糖尿病患者颈动脉内膜中层厚度(IMT)的影响。方法230例高血压患者根据病因分组,将高血压伴2型糖尿病患者分为合并糖尿病纽(100例),未合并糖尿病的高血压患者分为高血压组(130例)。测量血压,进行颈部动脉超声检查,测量颈动脉IMT,测定空腹血糖、胰岛素,并行单因素相关分析。结果合并糖尿病组患者颈动脉IMT为(0.67±0.08)mm,脉压为(61.93±14.55)mmHg(1mmHg=0.133kPa),均较高血压组明显升高[(0.50±0.03)mm,(55.76±12.98)mmHg,P〈0.01],合并糖尿病组患者胰岛素敏感指数(ISI,-3.68±0.82)较高血压组(-2.96±0.95)明显降低(P〈0.01)。相关分析显示,IMT与脉压呈正相关,与ISI呈负相关。结论脉压和胰岛素抵抗与高血压合并糖尿病患者颈动脉IMT密切相关。Objective To investigate the impact of office pulse pressure(PP) on carotid structure in elderly patients with essential hypertension(EH) and type-2 diabetes mellitus(DM). Methods 230 hypertensive patients were divided into 2 groups: type-2 DM complicated with EH (DM + EH) group, and EH group, All subjects underwent blood pressure measurement and ultrasonograpby of carotid arteries to measure the intima-media thickness(IMT). The biochemical parameters such as fasting glucose and insulin were tested. Results IMT was higher in DM+EH group that in EH group. There was a positive correlation between PP and IMT. Insulin sensitivity index (ISI) was negatively correlated to IMT. Conclusion PP and ISI are closely correlated to carotid IMT in DM+EH patients.
