缬沙坦对血液透析患者残余肾功能的保护作用

摘　　要：目的:探讨血管紧张素Ⅱ受体拮抗药(ARB)缬沙坦对维持性血液透析患者残余肾功能的保护作用。方法:将50例血液透析(HD)患者随机分为治疗组(缬沙坦组)和对照组,每组25例。治疗组予以缬沙坦胶囊80 mg,po qd,对照组予以ARB以外的降压药物。治疗前和治疗6、12月后测定患者24 h尿量、尿素清除指数(KT/V)、残余肾小球滤过率(eGFR)等评估指标。结果:研究结束后,两组患者24 h尿量均比研究前减少,但对照组较治疗组减少更为明显(P<0.05);两组eGFR均较研究前降低,但治疗组的降低速度显著低于对照组(P<0.05)。结论:缬沙坦对HD患者的残余肾功能具有一定的保护作用。Objective： To study the protective effect of angiotensin Ⅱ recepter blocker（ARB） Valsartan on residual renal function in maintaince hemodialysis patients. Method： Fifty cases with hemodialysis were randomly divided into treatment group and control group. Patients in treatment group were given Valsartan at a single daily dosage of 80m g. Blood pressure against agent other than ARB was taken in contral group. Twenty-four hour urine volume, KT/V and evaluation glomerular filtration rate （eGFR） were-monitored before therapy and 6 month, 12 month after therapy. Result： After the study was finished, 24-hour urine volume were decreased in all cases, which decreased more quickly in contral group（ P 〈0.05 ） ;eGFR in two groups were also decreased with which significantly decreased in cortrol group than that in treatment group. Conclusion： Valsartan can protect the residual renal function in HD patients.
