1996年输入病例Ⅲ型脊髓灰质炎野毒的分子病毒学检测

摘　　要：１９９６年３月和４月，我国云南省边境德宏傣族景颇族自治州人民医院收治了２名在缅甸勐古镇发病，来我国求治的急性弛缓性麻痹儿童。云南省卫生防疫站分别从２名病儿的粪便标本中分离到Ⅲ型脊髓灰质炎（脊灰）病毒；国家脊灰实验室经用ＰＣＲ－ＲＦＬＰ法检测，定为脊灰Ⅲ型野毒株，又经核酸序列测定，与ＳａｂｉｎⅢ型疫苗株有明显差异，但又有别于我国１９９５年新疆分离的Ⅲ型脊灰野毒株的序列。初步判定该２株脊灰Ⅲ型野毒株不属于中国本土的野毒株。Two type 3 wild polio cases were diagnosed in Yunnan Province in 1996 The patients resided in Myamar side near the border of China,and crossed the border seeking for doctor in Yunnan Province,after occurring paralysis in hometown They never received the OPV before and two type 3 polioviruses were isolated from their stool samples Using PCR-RFLP test the two isolates were diagnosed as wild polioviruses The VP1 sequence of 300 nucleotides of Yunnan isolates were analysed and compared with Sabin 3 vaccine strain There were 70 9% and 71 6% homogeneous respectively,so they were confirmed as wild type 3 polioviruses Since 1985 to 1994,type 1 wild poliovirus was the main epidemic strain causing poliomyelitis in whole China,and only one type 3 wild poliovirus was isolated in Xinjiang in 1993 We compared the Yunnan isolates with Xinjiang wild type 3 poliovirus,There were 81 3%-81 9% homogeneous The difference is over 15% with the China indigenous wild type 3 poliovirus Since 1995 we have not found any wild polio cases So we supposed the two Yunnan wild type 3 polioviruses were imported from Myamar,but the real source of the genotype of Yunnan isolates should be found out by comparing with the type 3 wild isolates from all of the world
