摘  要:目的:研究早期胃肠内营养对重型颅脑损伤康复治疗的临床疗效。方法:将114例重型颅脑外伤患者随机分成两组:A组57例,采用早期肠内营养;B组57例,早期肠外营养。比较两组入院第1、7、14天的血清蛋白、血红蛋白、转铁蛋白以及免疫球蛋白(IgGI、gAI、gM)和外周血淋巴细胞总数,同时观察两组胃肠道功能紊乱、院内感染发生率及GCS恢复情况。结果:A组在以上3个时间点的各项蛋白、免疫功能及GCS等指标明显优于B组,且A组胃肠道功能紊乱及院内感染发生率较B组明显减少。结论:早期适量肠内营养对重型颅脑外伤患者的临床康复治疗有一定疗效,并且可以降低胃肠道功能紊乱及院内感染发生率。Objective: To evaluate the effect of early enteral nutrition support in rehabilitative treatment of severe hea-injured patients. Methods: 114 cases of severe head-injured patients were randomly divided into two groups. Group A: 57 patients were given early enteral nutrition support;Group B: 57 patients were treated with early parenteral nutrition. The levels of serum albumin, hemoglobin, tmnsferrin, immunoglobulin (IgG, IgA, IgM) and total peripheral blood lymphocyte count were compared between the two groups in the first day, the seventh day, the fourteenth day after hospitalization,and the functional disturbances of gastrointestinal tract, nosocomial infection incidence, recovery condition of GCS were observed in the two groups. Results:Group A exceeded group B obviously in the levels of proteinum, the immune function and the GCS at the three time points. The incidences of functional disturbances of gastrointestinal tract and nosocomial infection were decreased obviousely in group A compared with group 13. Conclusion: Early enteral nutrition has certainly adjunctive effect in rehabilitative treatment of severe head injured patients, and can decrease the in eidenees of functional disturbances of gastrointestinal tract and nosocomial infection.
