2型糖尿病合并肺结核患者T淋巴细胞亚群和NK细胞的变化及其临床意义

摘　　要：目的探讨2型糖尿病合并肺结核患者外周血T淋巴细胞亚群和NK细胞的变化及其临床意义。方法对60例2型糖尿病合并肺结核患者(DM+LTB组)、30例2型糖尿病患者(DM组)、36例肺结核患者(LTB组)、32例健康者(NC组)监测外周血T淋巴细胞亚群(CD3+、CD4+、CD8+)和NK细胞的变化。结果DM+LTB组外周血CD3+明显低于DM组、LTB组和NC组(P<0.05),CD4+明显低于NC组(P<0.05),CD4+/CD8+、NK细胞明显低于DM组、NC组(P<0.05),CD8+显著高于DM组、NC组(P<0.05),CD4+、CD8+、NK与LTB组比较无统计学差异。DM组CD3+、CD4+、CD4+/CD8+显著低于NC组(P<0.05),CD8+、NK细胞水平与NC组比较差异无统计学意义(P>0.05)。LTB组CD3+、CD4+、CD4+/CD8+、NK细胞水平明显低于NC组(P<0.05),CD8+高于NC组(P<0.05)。结论2型糖尿病合并肺结核患者存在T淋巴细胞亚群的变化,存在着细胞免疫功能缺陷。Objective To explore the change and clinical significance of CD3^＋ , CD4^＋ , CD8^＋ and NK in type 2 diabetes mellitus with lung tuberculosis patients. Methods 60 type 2 diabetes mellitus with lung tuberculosis patients, 30 type 2 diabetes mellitus patients, 36 lung tuberculosis patients and 32 normal persons were screened and the activities of CD3^＋ , CD4^＋ , CD8^＋ and NK of them were measured, Results The activities of CD3^＋ in the 2 diabetes mellitus patients with lung tuberculosis were significantly lower than type 2 diabetes mellitus patients, lung tuberculosis and healthy persons（ P 〈 0.05）, CD4^＋ was significantly lower than NC group （ P 〈 0.05）, CD4^＋/CD8^＋ and NK were significantly lower than type 2 diabetes mellitus patients and healthy persons（ P 〈 0.05 ）, while CD8^＋ were significantly higher than type 2 diabetes mellitus patients and healthy persons（ P 〈 0.05）. There was no statistical difference between CD4^＋ , CD8^＋ , NK in the type 2 diabetes mellitus patients with lung tuberculosis and lung tuberculosis. The level of the type 2 diabetes mellitus with CD3^＋ , CD4^＋ , CD4^＋/CD8^＋ were signifieantly lower than healthy persons （ P 〈 0.05）, there is no statistical difference between CD8^＋ , NK in the type 2 diabetes mellitus patients with lung tuberculosis and healthy persons. The level of lung tuberculosis with CD3^＋ , CD4^＋ , CD4^＋/CD8^＋, NK in the patients were significantly lower than healthy persons（ P 〈 0.05）, CDs was higher than healthy persons（ P 〈 0.01 ）. Conclusion T lymphocytes were changed and cellular iwanuulty was deficient in type 2 diabetes mellitus patients with lung tuberculosis.
