摘 要:目的分析支气管哮喘(简称哮喘)患儿外周血CD4+T淋巴细胞表达烟碱样乙酰胆碱受体α7(nAChRa7)与相应的血清白细胞介素(IL)-4、干扰素(IFN)-γ水平的相关性,探讨nAChRa7表达与Th1/Th2平衡的关系及其在哮喘发病过程中的意义。方法选取2006年3月至2007年3月山东省立医院儿科门诊哮喘急性发作患儿30例为哮喘组,其中男16例,女14例,年龄3—9岁,平均(5±3)岁;儿科门诊查体健康儿童20名为健康对照组。取空腹静脉血,分离外周血淋巴细胞,并收取血清。用流式细胞仪检测淋巴细胞CD3、CD4和nAChRa7的表达。用ELISA法检测血清中IL4、IFN-γ的水平。结果哮喘组患儿外周血CD4+T淋巴细胞表达nAChRa7为(8.2±4.7)%,明显高于健康对照组的(5.9±1.9)%,差异有统计学意义(t=2.53,P〈0.05)。哮喘组血清中IL4水平为(11.6±5.5)ng/L,高于健康对照组的(5.8±3.0)ng/L,差异有统计学意义(t=5.42,P〈0.01);哮喘组IFN—γ水平为(5.7±2.6)ng/L,低于健康对照组的(11.7±7.8)ng/L,差异有统计学意义(t=4.11,P〈0.01)。外周血CD4+T淋巴细胞表达nAChRa7与血清IL-4水平呈正相关(r=0.688,P〈0.01),CD4+T淋巴细胞表达nAChRa7与血清IFN-γ水平呈负相关(r=-0.476,P〈0.01)。结论nAChRa7可能通过调节Th1/Th2细胞活性,使血清IL4水平升高、IFN-γ水平降低,参与哮喘气道炎症过程。Objective To investigate the relationship between the expression of neuronal acetylcholine receptor alpha 7 ( nAChRα7 ) and the role of nAChRα7 in the development of asthma by detecting the expression of nAChR α7 on CD4+ T lymphocytes in the peripheral blood and the level of cytokines IFN-γ and IL-4 in the blood serum from children with asthma. Methods Thirty patients in the asthma group and 20 healthy children in the normal control group were included. Peripheral blood was collected, and the serum was harvested. The expression of nAChRα7 on CD3 and CD4 cells were detected by flow cytometry. The levels of IFN-γ and IL-4 in the serum were detected by ELISA. Results The expression of nAChRα7 on CD4 T lymphocytes from children with asthma was higher than that from normal controls ( t = 2. 53, P 〈 0.05 ). Compared with the control group, the level of IL-4 in the serum of the asthma group was higher ( t = 5.42, P 〈 0.01 ) but the level of IFN-γ was lower ( t = 4. 11, P 〈 0.01 ). There were positive correlation between nAChRα7 and IL-4( r = 0. 688, P 〈 0.01 ) and negative correlation between nAChRα7 and IFN-γ( r = - 0. 476, P 〈 0.01 ). Conclusion nAChRα7 was shown to influence the balance of Th1/Th2 and it may play an important role in the development of asthma.
