局麻在老年无张力腹股沟疝患者疝修补术中的临床应用

摘　　要：目的比较老年人在局部神经阻滞麻醉及硬膜外麻醉下行无张力腹股沟疝修补术的疗效手术时间、平均费用等,探讨进一步提高腹股沟疝治疗疗效和减轻术后疼痛的方法。方法采用随机对照研究方法对我院2004年4月至2007年2月间进行的无张力腹股沟疝修补手术共220例患者,分为局麻组(110例)和硬膜外阻滞麻醉组(110例)。结果局麻组在患者费用(3628元),平均住院时间(2.6 d),术后恢复活动时间(2.2 h),并发症及术后复发率等方面具有优势,在手术时间、术后疼痛评分方面与对照组硬膜外麻醉下行无张力疝修补手术没有明显区别。结论老年人在局部神经阻滞麻醉下行无张力疝修补手术可明显降低手术费用、术后并发症的发生,是一种安全、简便、经济的方式,值得推广。Objective To study the efficacy of tension-free inguinal hernia repair of senile patients with local nerve blockade anaesthesia compared with epidural block anaesthesia including operative effect, operative time, costs in hospital etc. Methods Using random control study, 220 cases were selected from 2004.4 to 2002.2 in our hospital. All patients were randomly divided into 2 groups ： local nerve blockade anaesthesia group（ 110 cases） and epidural block anaesthesia group （110 cases）. Results In local anaesthesia group , costs in hospital（3628 yuan）, the average hospital stay （2.6 d） , the time to active of the patients （2.2 h）, complication and recurrence were less than that of the epidural block anaesthesia groups significantly. But there are no obvious difference of the two groups in operation time and pain feeling. Conclusions The tension-free inguinal hernia repair of senile patients under local nerve blockade anaesthesia can reduce the costs of operation , incidences postoperative complication significantly. It is a safe and economical method and is worthy to popularize.
