乙脑患儿血清和脑脊液中IL-18含量变化及其临床意义

摘  要:目的检测乙型脑炎患儿血清和脑脊液中白细胞介素-18(IL-18)的水平,并探讨其临床意义。方法采用双抗夹心酶联免疫吸附实验检测33例乙脑患儿血清及脑脊液中IL-18含量。结果乙脑患儿急性期血清及脑脊液中IL-18含量均明显高于对照组(P<0.01),差异有显著性。重型患儿血清及脑脊液中的IL-18含量明显高于普通型患儿(P<0.01),且恢复期血清中IL-18含量仍高于对照组(P<0.01)。普通型患儿血清恢复期IL-18含量与对照组比较无显著差异性(P>0.05)。结论IL-18在乙脑的发病过程中起到了一定的抗病毒的作用,但IL-18含量过高也与乙脑脑实质损伤程度密切相关,其含量增高可作为估计乙脑免疫状况、病情及预后的一项重要指标,并对乙脑免疫调节方面的治疗具有一定的参考价值。To detect levels of interleukin-18(IL-18) in patients with epidemic encephalitis B in children and to evaluate its clinical signiftcace, the levels of 1L-18 in serum and cerebrospinal fluid were measured in 33 patients with epidemic encephalitis B by a double-antibody sandwich ELISA. It was found that the level of IL-18 was significantly higher in fastigium of epidemic encephalitis B than that of controls group (P〈0.01). The average value of severe type in patients with epidemic encephalitis B was significantly higher than that of the common type (P〈0.01), and level of IL-18 was still higher than that of controls group in convalescence(P〈0.01). There were no significant difference between the common type group and controls patients at convalescence. These findings indicates that IL-18 may participate in the antiviral process of encephalitis B, but the high concentrations of IL-18 have also related with the degree of parenchymal lesions of brain. The elevation of IL-18 may be an important marker of the parenchymal lesions of brain in patients with epidemic encephalitis B, and have some reference value in the immunoloregulation of treatment for epidemic encephalitis B.
