老年高尿酸血症与胰岛素抵抗的相关性分析

摘　　要：目的探讨老年高尿酸血症(HUA)与胰岛素抵抗(IR)的关系,以期为心脑血管并发症二级预防措施实施提供依据。方法选择符合入选标准的患者204例,入选病例依血尿酸浓度(SUA)分为两组:HUA组(SUA≥430μmol/L)106例,正常对照组(NC,SUA<430μmol/L)88例。测定两组患者的空腹血糖(FPG)、空腹血清胰岛素(FIns);计算胰岛素抵抗指数(HOMA-IR)、胰岛素敏感性指数(ISI)及胰岛β细胞分泌功能指数(HOMA-β),分析两组结果的相关性。排除年龄及肥胖、高血压等相关因素。结果HUA组FPG、FIns、HOMA-IR水平明显高NC组,ISI明显低于NC组,差异均有统计学意义(均P<0.01),两组HO-MA-β差异无统计学意义(P>0.05)。SUA水平与FPG、FIns、HOMA-IR呈正相关,与ISI呈负相关(P<0.01)。结论老年高尿酸血症与IR密切相关,是代谢综合征的危险因素之一。Objective To explore the relations between hyperuricemia （HUA） and insulin resistance （IR） in senile patients. Methods Totally 204 patients, who were in accordance with the criteria were selected into this study. According to the concentration of serum uric acid （SUA）, they were divided into HUA group 106 （SUA t〉430 umol/L） and normal control （NC） groups 88 （SUA 〈 430 umol/L）. The levels of fasting plasma glucose （FPG）, fasting insulin（Fins） were determined,and The homeostasis model assessment of insulin resistance index （HOMA- IR）, insulin sensitive index（ISI） and Homa β-cell function index（HOMA-β） were calculated in both groups, the correlation were analyzed. After adjusting the effects of the age and correlative factors of obesity, hypertension etc, there existed a close relationship between HUA and IR. Results There were very significant differences （ P 〈 0. 01 ） in FPG, Fins, HOMA-IR and SUA between HUA group and NC group. The levels of FPG, Fins, HOMA-IR and SUA were higher in HUA group than in NC group. ISI was obviously lower in HUA group than in NC group. There was not signif- icant difference（P 〉 0.05 ） in HOMA-β between HUA and NC groups. Analysis showed that SUA was positively correlated with FPG, Fins, HOMA-IR, and negatively correlated with ISI （P 〈0. 01 ）. Conclusions HUA is one of risk factors for the metabolic syndrome（MS） ,and IR is closely related to HUA.
