摘  要:目的探讨实时三维超声心动图双平面成像评价风湿性二尖瓣狭窄瓣口面积的可行性和准确性。方法选择30例拟行二尖瓣置换术的风湿性二尖瓣狭窄患者,术前应用二维超声、实时三维双平面成像观察二尖瓣病变,并测量二尖瓣瓣口面积(MVA),以术后描记法测量的二尖瓣标本瓣口面积为金标准进行比较分析。结果二维超声测量二尖瓣瓣口面积(MVA2DE)、实时三维双平面成像测量二尖瓣瓣口面积(MVART-3DE)与描记法测量二尖瓣标本瓣口面积(MVAOP)之间差异无统计学意义(P>0.05),MVA2DE、MVART-3DE与MVAOP的相关性分别r=0.93,r=0.94。BlandAltman分析图示MVART-3DE与MVAOP一致性良好。结论实时三维超声心动图双平面成像能同时实时显示二尖瓣口的胸骨旁左室长轴和短轴两个相互正交的切面,有利于操作者实时、快速、准确地切取到最小面积瓣口平面,准确反映瓣膜狭窄的真实程度。Objective To explore the accuracy and feasibility of real-time three-dimensional echocardiography biplane imaging(RT- 3DE)for assessing stenotic mitral valve orifice area(MVA). Methods Thirty inpatients with rheumatic mitral stenosis underwent mitral valve replacement surgery. The maximal mitral valve orifice area( MVA) in diastole was measured before operation by two dimensional echocardiography(2DE)and RT-3DE,and compared with that by determining mitral specimen after operation. Results There was no statistical difference between MVA2DE and MVAOP,between MVA RT-3DE and MVAop(P 〉0.05). The correlation coefficients of MVA2DE, MVART-3DE and MVAOP were 0.93 and 0.94, respectively. Bland Altman analysis demonstrated that MVART-3DE was well consistent with MVAop. Conclusion RT-3DE can conveniently, accurately and reliably quantify mitral stenosis.
