摘 要:目的观察丙泊酚不同处理方式对离体大鼠缺血再灌注心肌的保护效应,探索其最佳给药途径。方法对大鼠离体心脏行改良Langendorff灌流,将32只SD大鼠随机分为4组,每组8只。A组持续正常K-H液灌注40min后全心停灌30min,再灌注60min;B组平衡20min后用含50μmol/L丙泊酚K-H液灌注10min,再用K-H液冲洗10min,缺血与再灌注处理同A组;C组K-H液灌注40min,全心停灌30min后再灌注时先用含50μmol/L丙泊酚K-H液灌注10min,再用K-H液灌注50min;D组平衡20min后用含50μmol/L丙泊酚K-H液灌注20min,全心停灌30min,再用含50μmol/L丙泊酚K-H液灌注60min。观察缺血再灌注期间HR、LVDP、LVEDP、±dp/dtmax变化及灌注末心肌组织中丙二醛(MDA)含量、心肌超氧化物歧化酶(SOD)活性。结果再灌注后,与A组相比,B、C、D组HR、LVDP、LVEDP、±dp/dtmax均有显著变化(P<0.01),D组各项血流动力学指标均优于B、C组(P<0.05)。与A组相比,B、C、D组SOD活性均明显增高,MDA含量减少,差异有统计学意义(P<0.01),以D组更为显著。结论丙泊酚对离体大鼠缺血再灌注心肌有保护效应,缺血前后持续以含50μmol/L丙泊酚K-H液灌注保护效应最为明显。Objective To investigate the protective effects of different treatment modus of propofol on isolated rat hearts against ischemia reperfusion injury,and to explore the optimal administration route. Methods 32 SD rat hearts were isolated and perfused with oxygenated Krebs-Hensleit (K-H) solution in a Langendorff apparatus. The isolated hearts were randomly divided into 4 groups (8 hearts each group):ischemia-reperfusion group (A group) in which the isolated hearts were subjected to 30 rain no-flow global ischemia followed by 60 min reperfusion after 40 rain stabilization; propofol pretreatment group (B group) in which the hearts were perfused with K-H solution containing propofol (50μmol/L) for 10 min followed by 10 rain propofol washout before ischemia-reperfusion; propofol posttreatment group (C group) in which the laearts were perfused with K-H solution containing propofol (50μmol/L) for 10 min after global ischemia;propofol treatment group (D group) in which the hearts were perfused with K-H solution containing propofol (50 μmol/L) for 20 rain before isehemia and after global ischemia. The left ventrieular develop pressure (LVDP), the left ventricular end-diastolic pressure (LVEDP), the heart rate ( HR), and ± dp/dtmax were monitored continuously. At the end of reperfusion myocardial specimen was obtained from left ventrical and homogenized for determination of the activities of superoxide dismutase(SOD) and contents of malondialdehyde (MDA). Results Compared with those of group A, HR,LVDP, LVEDP and ± dp/dtmax were significantly improved, the activities of SOD increased, while the contents of MDA decreased in group B,C,and D,especially in group D (P〈0.01). Conclusion Propofol can protect isolated rat heart against isehemia-reperfu- sion injury. Treatment of 50μmol/L propofol before and after global ischemia can improve function of the isolated rat heart after ischemia-reperfusion injury.
