Tei指数监测蒽环类药物对肿瘤患者左心功能的影响

摘　　要：目的探讨心肌做功指数(myocardial performance index,MPI 又称 Tei index)在评价蒽环类药物(an-thracycline,ATC)对肿瘤患者左心功能早期损害方面的临床应用价值。方法测定并比较50例肿瘤患者 ATC 化疗前和化疗后6个月常规超声心动图参数及 Tei 指数的变化。结果与化疗前相比,左心室等容舒张时间(IRT)延长(P<0.05),左心室 Tei 指数明显升高(P<0.01)。而左心内径及收缩功能指标(EF、FS)化疗前后无显著性差异,仅14例(28%)患者化疗后出现一项或多项常规左室舒张功能指标异常(E 峰减低,A 峰升高,E/A<1)。结论 Tei 指数能更早、更敏感地评价 ATC 对肿瘤患者左心功能的损害。Objective To explore the clinical value of Tel index assessing left ventricular funtion in malignant patients receiving cardiotoxic anthracycllin chemotherapy. Methods 50 malignant patients were examined by eehocardiography and Tei index before anthracycline chemotherapy and six months after therapy. Results Left ventricular isovolumic relaxation （IRT）were significantly prolonger（ P 〈 0. 05 ） after chemotherapy, and left ventricular Tel index were significantly higher（ P 〈 0.01 ）. Parameters acquired from common echoeardiography showed that the regression of diastolic function of left ventricle appeared in 14 patients （28 % ）. While left ventricular systolic function （EF）post-chemotherapy had no statistic significance compared with that pre-chemotherapy（ P 〉0. 05）. Conclusion Compared with common echocardiography , Tei index is more sensitive in detecting abnormalities of left ventricular function.
