摘  要:目的探讨糖耐量异常合并高血压患者24小时动态脉压(24hMPP)与靶器官损害之间的关系。方法入选56例糖耐量异常合并高血压病患者(高血压组)和34例糖耐量异常、血压正常患者(对照组),分别进行24小时平均脉压(AMBP)、超声心动图、颈动脉超声及24h尿微量白蛋白定量(UAE),对相应数据进行统计学分析。结果两组在年龄、性别、体重指数、糖化血红蛋白和血脂等方面差异无统计学意义(P>0.05),高血压组的24hMPP、24hSBP和24hMDBP明显高于对照组(P<0.01),该组的LVM1、CCA-IMT和UAE高于对照组,差异有显著性(P<0.01)。高血压组的24hMPP与LVM1、CCA-IMT和UAE呈正相关(r分别为0.39、0.41、0.38,P<0.01)。结论24h动态脉压对糖耐量异常合并高血压病患者的靶器官损害有预测价值,行24小时动态脉压测定,控制脉压,将有利于对靶器官损害的早期发现和逆转。Objective To study the relationship between ambulatory pulse pressure and subclinical target organ damage in patients with both impaired glucose tolerance and hypertension.Methods 80 patients with impaired glucose tolerance (IGT) were enrolled,46 patients with hypertension as cases and 34 normotensive as controls.All cases and controls were measured of ambulatory blood pressure monitoring, echocardiograph, carotid ultrasonograph, micro albuminuria and parameters calculated including 24h oursam bulatory pulsep ressure(24hMPP), left ventricular massin dex(LVMI), carotid intima-media thickness(CCAIMT) and urinary albumin excretion (UAE).And then,the parameters from the two groups were compared with each other.Results There were no significant difference between two groups in age, gender, body mass index(BMI), HbAle and plasma lipids. 24hMPP, 24hSBP, 24hMDBP, LVMI, CCA-IMT and UAE in hypertension group were significantly higher than those in control group 0a〈0.O1). 24hMPP in hypertension group was positively correlated with LVMI, CCA-IMT and UAE (r=0.38, 0.43, 0.39, respectively.P 〈 0.01 ).Conclusion Ambulatory pulse pressure could be of prognostic value in estimating the degree of target organ demage in IGT patients with hypertension. Monitoring 24hMPP, and decrease pulse pressure in IGT patients with hypertensive may be beneficial to early detection and reversion of target organ damage.
