摘 要:目的分析呼吸科重症监护病房(RICU)患者感染相关危险因素,并对病原菌的耐药性进行研究。方法对医院2005年1-12月收住RICU的全部患者进行调查监测,采用API细菌鉴定系统对病原菌进行鉴定,K-B法药敏试验。结果265株病原菌中以革兰阴性杆菌为主,占74.34%,其中以铜绿假单胞菌、鲍氏不动杆菌和肺炎克雷伯菌占优势,分别占所有分离菌的28.68%、23.02%和5.66%;革兰阳性球菌以金黄色葡萄球菌居多,占所有分离菌的4.53%,肺炎克雷伯菌和大肠埃希菌对亚胺培南、美罗培南100.00%敏感,铜绿假单胞菌和鲍氏不动杆菌对亚胺培南、美罗培南的耐药率分别为54.17%、50.70%和23.73%、37.93%;在分离的革兰阳性球菌中,未检出对万古霉素耐药株。结论应充分认识RICU患者感染相关危险因素,重视病原学资料,合理应用抗菌药物。OBJECTIVE To investigate the associated risk factors for the infected patients in respiratory intensive care unit (RICU) and to study the resistance of isolates. METHODS To study the bacterial spectrum and drug resistance test of all the patients with hospital acquired infection from Jan to Dee 2005. Identification and antimicrobial susceptibility were determined with the methods of API and K-B,respectively. RESULTS A total of 265 strains were isolated from 117 patients, the rate of isolation of Gram-negatives was the highest (74.3%), the species were predominated by Pseudomonas aeruginosa , Acinetobacter baumannii and Klebsiella pneumoniae , the rates of isolates were 28. 68%, 23.02% and 5.66%, respectively. The predominated species of Gram-positives was Staphylococcus aureas. Gram-negative bacilli were susceptible to imipenem and meropenem, there were not vancomycin-resistant strains. CONCLUSIONS It is important to realize the associated risk factors of infected patients in RICU and to promote the rational use of antimicrobial agents.
