改良膀胱穿刺引流在经尿道前列腺电切术中的应用(附32例报告)

摘　　要：目的:探讨高龄、大体积前列腺和/或合并高血压、心衰等高危患者前列腺手术切除方式的选择及其安全性。方法:对32例高危前列腺增生患者经尿道置入电切镜,在TUR显示器监视下,将腹腔镜10mm Trocar自耻骨上两横指穿刺进入膀胱腔内,去除管芯,置入F26菌形尿管并固定外接无菌引流袋。然后,在持续低压灌洗状态下行前列腺电切术。结果:该组32例患者手术均安全顺利完成,无一例出现TUR综合征及其它严重并发症,其可耐受手术时间明显延长。结论:改良膀胱穿刺引流应用于高危患者行经尿道前列腺电切手术,能增加手术的安全性和成功率,减少并发症的发生。Objective：To investigate the effects and safety of the surgical treatments for benign prostatic hyperplasia（BPH）patients with high risk factors.Methods：Transurethral resection of the prostate（TURP）was performed after the suprapubic bladder puncture with our modification under the monitoring of TUR display.Results：The operations of the 32 cases were successful.No TUR-syndrome and other serious complications were observed in all the cases and the operation time that these patients cluld endure was prolonged.Conclusion：The application of modified puncture of bladder to transurethral resection of the prostate is effective and safe in treating BPH patients with high risk factors.It can increase safety and successful rate of operation and decrease complications of the patients.
