摘  要:目的比较三维稳态进动快速成像(3D FIESTA)和三维快速恢复快速自旋回波T2加权成像(3D FRFSE T2WI)对12对颅神经的显示,探讨颅神经的最佳MR扫描序列。资料与方法对5名志愿者和17例主诉头晕、耳鸣和听力下降或丧失的患者采用3D FIESTA和3D FRFSE T2WI两种扫描序列进行扫描,并由两名神经放射学医师根据528支颅神经显示的清晰程度分为以下3个等级:清晰、较清晰、不清晰,前两者定义为显示,后者定义为未显示。结果颅神经在3D FRFSE T2WI和3D FIESTA的显示率分别为:嗅神经(84%、80%),视神经(100%、100%),动眼神经(82%、100%),滑车神经(5%、39%),三叉神经(100%、100%),外展神经(45%、98%),面神经(100%、100%),前庭蜗神经(100%、100%),后组颅神经复合体(舌咽神经、迷走神经、副神经)(70%、100%),舌下神经(7%、86%)。对两种序列显示颅神经有差异者进行卡方检验,除嗅神经和动眼神经的P值分别为0.58和0.009外,其余P<0.001。结论在显示脑池段颅神经,尤其是细小的颅神经时3D FIESTA序列明显优于3D FRFSE T2WI序列。Objective To compare 3D FIESTA with 3D FRFSE T2WI MRI sequences in the evaluation of cranial nerves Ⅰ-Ⅻ. Materials and Methods 3D FIESTA and 3D FRFSE T2WI were used in 5 volunteers and 17 patients with complaint of vertigo,tinnitus and hearing loss. 528 nerves were evaluated. Two radiologists independently divided the imaging findings into 3 grades: 1 ( completely visualized), 2 ( partially visualized), and 3 ( not visualized). The first two grades were defined as visualization and the last grade was no visualization. Results The rate of visualization of these cra- nial nerves with 3D FRFSE T2WI and 3D FIESTA sequences were as follows respectively: nerve Ⅰ (84% and 80% ,P = 0.31 〉0.05);nerve Ⅱ/(100% and 100%);nerve Ⅲ(82% and 100%,P〈0.001);nerve Ⅳ(5% and 39%,P〈 0.001 ) ;nerve Ⅴ ( 100% and 100% ) ; nerve Ⅵ (45% and 98%, P 〈 0. 001 ) ; nerve Ⅶ ( 100% and 100% ) ; nerve Ⅷ ( 100% and 100% ) ; nerve Ⅸ - Ⅺ complex (70% and 100%, P 〈 0.001 ) ; nerve Ⅻ( 7% and 86%, P 〈 0.001 ). Conelusion 3D FIESTA sequence is superior to 3D FRFSE T2WI sequences in the evaluation of cisternal parts of cranial nerves, especially tiny cranial nerves.
