摘  要:目的探讨连续性血液净化对MODS患者内毒素受体及其信号转导的影响。方法30例MODS患者随机分为低容量组和高容量组,分别检测治疗前、治疗后6h、1天、3天血浆TNF-α、IL-6水平,并观察血液净化治疗前后中性粒细胞(PMN)的CD14mRNA、CD14蛋白表达以及NF-κB活性改变。结果血液净化对血浆TNF-α无影响,IL-6水平较治疗前明显降低(P<0.05);MODS患者PMN膜CD14mRNA表达、PMN膜CD14蛋白表达、NF-κB活性经血液净化治疗后明显降低(P<0.01)。结论连续性血液净化能够削弱MODS患者内毒素受体表达及其信号转导。Objective To investigate the effct of continuous blood purification (CBP) on endotoxin receptor and signal transduction of patients with multiple organ dysfunction syndrome( MODS). Methods Thirty patients with MODS were selected and treated with low volume and high volume haemofiltration. Plasma level of TNF - α ,IL -6 were messured before and after CBP at 6h, 1 d,3d. Express of PMN CD14mRNA,CD14 protein and activity of NF - κB were also observed. Results CBP had no effect on TNF - α, and plasma IL - 6 decreased significantly (P 〈0.05 ). Express of PMN CD14mRNA, CD14 protein and activity of NF - κB decreased significantly after CBP (P 〈 0.01 ). Conclusion CBP could impair the express of endotoxin receptor and signal transfer of patients with MODS.
