摘 要:目的分析评价杭州市城区和县(市)两种不同的肺结核病归口管理模式。方法对城区和县(市)2002~2007年肺结核病报告登记资料、报表、登记本及督导记录进行对比分析。结果城区和县(市)活动性肺结核报告率分别为92.29%~99.81%和90.05%~99.08%;转诊率分别为88.31%~99.91%和88.32%~100%;转诊到位率分别为61.62%~97.82%和70.25%~93.95%;登记率分别为71.38/10万~104.34/10万和53.79/10万~69.84/10万;新登涂阳率分别为19.06/10万~35.53/10万和20.98/10万~26.42/10万;新涂阳登记率分别为16.10/10万~31.34/10万和16.44/10万~23.45/10万;初治涂阳肺结核治愈率分别为74.67%~90%和78.76%~90.04%;复治涂阳治愈率分别为45.24.37%~79.17%和47.25%~84.08%。经统计学处理两种模式管理下的转诊和转诊到位率、新登率、新登涂阳率、新涂阳率指标均呈现上升趋势;两种模式新涂阳登记率和初治涂阳治愈率无显著性差异。结论杭州市城区和县(市)因地制宜采用不同归口管理模式,传染性肺结核发现率和治愈率都显著提高,结核病管理工作均取得明显成效。Objective To analyze and evaluate two models of centralized management on tuberculosis cases from urban area and counties of Hangzhou. Methods Contrast the tuberculosis case notification notes, report forms, case registration books and supervision notes from 2002 to 2007 of urban area and counties of Hangzhou. Results The case notification rate of urban area and counties was 92.29% -99.81% and 90. 05% -99. 08% respectively;the referral rate was 88.31% -99.91% and 88.32% - 100% respectively;atrive rate of referral cases was 61.62% - 97.82% and 70.25% - 93.95% ; registration rate was 71.38/100000 - 104. 34/100000 and 53.79/100000 - 69.84/100000 respectively; Positive rate of new registration cases was 19.06/100000 - 35. 53/100000 and 20.98/ 100000 - 26.42/100000 respectively ; registration rate of new smear positive cases was 16. 10/100000 - 31.34/100000 and 16.44/100000 - 23.45/100000 respectively;Cure rate of new smear positive cases was 74.67% -90% and 78.76% -90.04% respectively;cure rate of re - treatment smear positive cases was 45.24.37% - 79.17% and 47.25 % - 84.08% respectively. The referral rate,arrive rate of referral cases, registration rate of new cases, positive rate of new registration cases and smear positive rate of new cases from both models show statistical rising trend. While the registration rate of new smear positive cases and cure rate of new smear positive cases from them didn' t showed statistically significant difference. Conclusion Detection rate and cure rate of Hangzhou are obviously elevated when two different centralized management models were applied to urban area and counties respectively. The tuberculosis management of Hangzhou made a re- markable progress.
