孕前期、住院分娩妇女口服碘油胶丸后不同时期尿碘、乳碘排泄分泌情况观察

摘　　要：目的:掌握孕前期、住院分娩妇女口服碘油胶丸后不同时期尿碘、乳碘排泄分泌规律。方法:选择尿碘小于150μg/L孕前期、住院分娩妇女,用酸消化砷铈接触法(W S/T107-1999)对对照组和服药组孕前期、住院分娩妇女的尿液、乳汁进行尿碘、乳碘检测。结果:尿碘小于150μg/L孕前期、住院分娩妇女服药组服药后第1天尿碘中位数分别为80908、81723μg/L,乳碘中位数73645μg/L,21 d之内尿碘、乳碘排泄、分泌最快,第1天尿碘、乳碘排泄、分泌达到高峰。1个月时排泄、分泌变缓,至第9个月尿碘、乳碘中位数分别为178.3、189.33μg/L,乳碘中位数176.44μg/L,第3个月至第9个月尿碘、乳碘排泄、分泌趋于稳定且逐渐下降到正常范围。结论:口服碘油胶丸后尿碘、乳碘排泄、分泌规律及保持正常水平的时间与育龄妇女怀孕到妊娠时间,新生儿最佳母乳喂养时间相一致。因此,对缺碘孕前期、住院分娩妇女口服0.2 g碘油胶丸可达到较好的干预效果。Objective：Thoroughly learning the rules of iodine excretion and secretion in urine and milk in different phases after women in prophase of pregnancy and in delivery take capsules of iodized oil. Methods： Selecting women in prophase of pregnancy and in delivery with iodine index in urine less than 150 μg/L to inspect the iodine index in their urine and milk with contact method of digesting As and Ce by acid （ WS/T107 - 1999 ） in two women groups including contrast group and medicine - taken group. Results：The medians of 1 d iodine index in urine of women in medicinetaken group with iodine index in urine less than 150 μg/L are 80908,81723 μg/L and the median of iodine index in milk is 73645 μg/L, the iodine in urine and milk excrete and secrete fastest within 21 d and the excretion and secretion of 1 d iodine in urine and milk reaches peak value. The excretion and secretion become slow within one month, the medians of iodine index in urine and milk are 178.3,189. 33 and 176. 44 μg/ L respectively by the ninth month, the excretion and secretion of iodine in urine and milk go into stability and decline to normal range from the third month to the ninth month. Conclusion ：The rules of excretion and secretion of iodine in urine and milk after taking soft capsules of iodized oil orally and the time of normal level are consistent with the time of women＇s pregnancy and the best milk feeding time for the newly - born babies. Therefore, to the women of iodine deficiency in prophase pregnancy and in delivery, they could take 0. 2 g soft capsules of iodized oil orally for a preferable action.
