摘 要:目的了解福建省人体钩虫感染的分布,分析其易感因素。方法采用改良Kato-Katz涂片法检查;采用1∶4的个体匹配方法,选择80例钩虫感染者和320例健康的同年龄组同性别的对照组进行病例对照研究,进行条件Lo-gistic回归模型分析。结果共调查20 195人,钩虫感染3 985人,感染率19.73%,女性(21.16%)高于男性(18.21%),轻度感染者占70.44%。感染高峰年龄为成年人。以闽南感染率最高(29.05%)。回归分析表明,家庭人均收入、饮用水源类型、便后洗手、喝生水和赤脚下地劳动等5个因素与钩虫感染有关(P<0.05)。结论福建省钩虫感染率呈总体下降趋势,但种植经济作物为主的地区仍居高不下,是钩虫防治重点;成年人是钩虫感染的主要人群;影响钩虫感染的主要危险因素是家庭人均收入、饮用水源类型、便后洗手、喝生水和赤脚下地劳动。Objective To understand the distribution and risk factors of hookworm infection in Fujian Province. Methods The modified Kato-Katz thick smear technique was used to examine eggs of hookworm. A case control study including 80 cases of hookworm and 320 controls was carried out using the paired design (1 : 4), and a total number of 13 factors were analyzed by conditional logistic regression. Results 3 985 of 20 195 persons were found to be infected with hookworm with the infection rate of 19. 73%, and the male and female infection rates were 18.21% and 21.16% ,respec- tively(P〈0.05). A total of 70.44 % cases were low intensity of hookworm infection, and the highest infection rate was in the age group of adult. The analysis of geographical distribution showed that the highest infection was in the Southern. The regression analysis revealed that the main five factors concerned with hookworm infection included average income, drinking water, washing hand after defecate, drinking unboiled water and barefoot working in the fields (P〈0.05). Conclusion The infection rate of hookworm in Fujian Province was shown downtrend in general, but the infection rate was still high in the area of economic crops. Prevention and cure emphasis was still soil-transmitted parasitosis in our province, and adult were high risk population. The risk factors of hookworm infection were family average income, drink ing water, washing hand after defecate, drinking unboiled water and barefoot working in the fields.
