摘 要:目的探讨多层螺旋CT诊断耳硬化症的价值。方法收集经临床、手术证实为耳硬化症、且进行了多层螺旋CT扫描病例共34例。扫描层厚为0.625mm,进床速度为1.2mm/rot。双侧分别小视野骨算法重建,重建视野为9.6cm,重建层厚为0.625mm,重建间隔为0.2mm。结果34例共64耳显示异常。耳硬化症的多层螺旋CT表现可分为窗型、耳蜗型及混合型。窗型54耳,仅前庭窗异常47耳,表现为前庭窗扩大或狭小;前庭窗及蜗窗同时受累共7耳。耳蜗型5耳,仅累及耳蜗周围迷路骨质,表现为耳蜗骨迷路边缘不整,呈条片状密度减低或双环状改变。混合型5耳,同时累及耳蜗、前庭窗、蜗窗及内耳迷路其它部分。结论多层螺旋CT是诊断耳硬化症有价值的方法。Objective To evaluate the value of multi - detector CT C MDCT) in diagnosis of otosclerosis. Methods 34 cases with otosclerosis confirmed by operation underwent MDCT scanning in spiral pattern. The slice thickness was 0. 625 mm, table speed was 1.2 mm/ rot. The temporal bones were reconstructed in a small FOV by bone arithmetic method. Reconstructed field of view was 9.6 cm, reconstructed slice thickness was 0. 625 mm and reconstructed interval space was 0. 2 mm. Results 64 temporal bones were abnormal. 47 ears manifested enlargement or narrowing only in vestibular window. 7 ears displayed abnormality in vestibular and cochlear windows simultaneously. 5 ears manifested abnormal bone labyrinth only around the cochlea, exhibited striped decreased attenuation or double circle. 5 ears displayed abnormality in bone labyrinth around the cochlea and in vestibular and cochlear windows simultaneously. Conclusion MDCT is a valuable method for diagnosis of otosclerosis.
