食管癌切除食管胃机械吻合术中国产与进口吻合器对比研究

摘　　要：目的:比较食管癌切除食管胃机械吻合术中国产吻合器与进口吻合器疗效,评价国产吻合器的应用价值。方法:回顾性研究食管癌贲门癌患者125例,按术中行器械吻合使用器械分两组:国产器械吻合组(A组)78例,进口器械吻合组(B组)47例。比较两组病人食管胃吻合时间、总手术时间、吻合口瘘、吻合口狭窄、吻合口出血、呼吸循环并发症发生率、围手术期死亡率、住院时间、住院费用等。结果:B组平均胃食管吻合时间、手术时间、吻合口瘘发生率、吻合口狭窄发生率、住院天数及住院费用分别为(11±3)min、(136±21)min、2.1%、4.2%、(9±3)天、(13671.80±976.70)元,A组分别为(13±5)min、(139±23)min、3.8%、5.1%、(10±2)天、(11013.72±876.56)元;两组胃食管吻合时间、手术时间及住院时间差异无显著性。费用B组增加。Objective ：To compare the efficacy of domestic stapler and imported stapler in the surgical treatment of esophageal car- cinoma, and evaluate the clinical application value of using domestic gastroesophageal stapler.Methods：From Jan. 2002 to Jan.20（O, 125 patients who underwent surgical treatment of esophageal carcinoma in the people＇s hospital of Guangxi zhuang autonomous region were studied retrospectively. The condition during and after the operations as well as the complications correlated with anastomoses were analyzed, All cases were grouped by the way of suturing.78 cases used with domestic stapler （A） and 47 cases used with imported stapler （B）.Results：The average time of gastroesophagostomy, operative time, postoperative hospital day, cost of hospitalization, anastomotic leakage rate and anastomotic constriction rate were（11±3）min, （136±21）min, （9±3）days, （13671.80±976.70）yuan,2.1%（1/47） and 4.2% （2/47） in imported staple group A,while in domestic stapler group B（ 13±5）rain, （139±23）rain, （10± 2）days, 11013.72±876.56 yuan, 3.8% （3/78） and 5.1%（4/78） respectively. Between domestic stapler group A and imported stapler group B,there were no statistical differences in the average time of gastroesophagostomy,operative duration,pestoperative hospital day,anastomotic leakage rate, anastomotic constric- tion rate, anastomotic hemorrhage, operative mortality rate and complication rate,but the cost of hospitalization was less in the domestic stapler group.Conclusion：There were no differences in the safety and clinical outcome between the domestic stapler and imported one, while the domestic stapler cost was less.
