摘 要:目的了解下呼吸道革兰阴性杆菌感染构成及耐药性,为临床合理使用抗菌药物提供科学依据。方法对2007年1月—2008年5月在内蒙古医学院附属人民医院住院及门诊下呼吸道感染患者合格痰标本进行细菌学培养,细菌鉴定采用法国生物梅里埃公司API-20E检定系统,用K-B法做药敏监测。结果共分离到612株致病菌,其中革兰阴性杆菌502株(82%),主要以肺炎克雷伯菌、大肠埃希菌、铜绿假单胞菌、阴沟肠杆菌和费劳地枸橼酸杆菌多见。体外药敏结果显示:革兰阴性杆菌除铜绿假单胞菌外,亚胺培南敏感性最高(100%),最低为氨苄西淋(均值15.6%),铜绿假单胞菌对阿米卡星敏感性最高(92.6%)。结论医院下呼吸道感染以革兰阴性杆菌为主,不同的细菌对同一抗菌药物有不同的敏感性,应依据药敏试验结果合理选择抗菌药物。[Objective]To investigate the infection composing and drug resistance of gram-negative bacillus from lower respiratory tract, provide the scientific evidence for rational application of antibiotics in clinic. [ Methods] The qualified sputum samples from inpatients and outpatients with the lower respiratory infection in the Affiliated People's Hospital of Inner Mongolia Medical College from January,2007 to May,2008 were detected with bacteriological culture. The bacteria were identified by API -20E identification system of France Maryai company. The drug sensitivity was tested by K-B methods. [Results] 612 strains of pathogenic bacterium were isolated, and 502 strains ( 82% ) were gram-negative bacillus. Among them, K. peumoiae J , E. eolil , P. aeruginosa, E. cloaeacand and C. freundii was common. Drug sensitivity test of vitro showed that except P. aeruginosa, the sensitivity of imipenem was the highest ( 100% ), and that of ampicillin was the lowest (15.6% ). The sensitivity of P. aeruginosa was the highest for amikaein (92.6% ). [ Conclusion] Gram-negative bacillus was the dominating bacteria in lower respiratory infection. Different bacterium had different sensitivity for the same antibacterials. The antibioties should be chosen according to the results of drug sensitivity test.
