2型糖尿病与胃黏膜损伤的相关性分析

摘　　要：目的:观察高血糖对胃黏膜的影响并分析其作用机制。方法:2型糖尿病组122例,糖尿病前期组30例,非糖尿病组30例,观察3组患者胃黏膜的镜下及病理表现。结果:内镜下可见胃黏膜异常患者,2型糖尿病组占91.8%(112例),糖尿病前期组占40.0%(12例),非糖尿病组占20.0%(6例),2型糖尿病组与糖尿病前期组及非糖尿病组比较差别有统计学意义(χ2分别为81.6和110.5,P<0.05),糖尿病前期组与非糖尿病组比较差异有统计学意义(χ2=36.8,P<0.05)。糖尿病病程与非萎缩性胃炎呈负相关(rs=-0.784,P<0.05),糖尿病病程与萎缩性胃炎、肠化、异型增生呈正相关(rs分别为0.815、0.780和0.745,P<0.05)。接受3种不同降糖治疗方法的2型糖尿病患者及未用药的2型糖尿病患者的非萎缩性胃炎和萎缩性胃炎及肠化、增生的发生率比较无统计学意义(χ2=1.792,P>0.05)。结论:高血糖状态可损伤胃黏膜,糖尿病病程越长胃黏膜萎缩越明显,降糖治疗方法与胃黏膜损伤程度无明显关系,对糖尿病患者要注意对胃黏膜的保护。Objective：To observe the effect of hyperglycemia on gastric mucosal and to investigate its potential mechanism in diabetic patients.Methods：The gastric mucosa of 122 cases with type 2 diabetes,30 cases with pre-diabetes and 30 cases without diabetes were observed by electric microscope and pathologic examination.Results：The gastric mu-cosal lesions were found in 91.8% of type 2 diabetes,40.0% of pre-diabetes and 20.0% of control group under microscope.There was significant difference between pre-diabetes and control group（χ2=36.8,P 〈 0.05）,meanwhile significant differ-ence was found between type 2 diabetes and pre-diabetes group（χ2 =81.6,P 〈 0.05）,and significant difference between type 2 diabetes and control group（χ2=110.5,P 〈 0.05）.There was a negative correlation between non-atrophic gastritis and the duration of diabetes（rs = -0.784,P 〈 0.05）.There was a positive correlation among the duration of diabetes,atrophic gastritis,intestinal metaplasia,and dysplasia（rs =0.815,0.780 and 0.745,P 〈 0.05）.Conclusion：The long term hyper-glycemia may harm gastric mucosa.There were positive correlation between gastric mucosal lesions and the duration of dia-betes.The longer the diabetes duration,the severer the gastric mucosal lesions will be.It is important to protect the gastric mucosa in patients with diabetes.
