病友俱乐部对慢性乙型肝炎患者健康教育效果分析

摘　　要：目的探讨采用病友俱乐部的健康教育方式对慢性乙型肝炎(简称乙肝)患者进行健康教育的效果。方法选择市区机关企事业单位体检人群中查出的乙肝患者222例,随机分为干预组(112例)和对照组(110例),干预组采用病友俱乐部的方式进行系统的乙肝相关知识教育,并开展一系列社会支持服务,为期1年。而对照组采用常规的健康教育方式。结果干预组病例的医嘱执行情况明显改善,规律用药、科学营养、戒酒、合理作息、定期复查分别由干预前的71.82%、60.91%、45.45%、63.64%和68.18%提高到干预后的96.43%、97.32%、92.86%、83.93%和97.32%。在生活质量方面,干预组躯体健康维度、物质生活维度、心理健康维度、社会功能维度得分分别由教育前45.07±5.23分、33.36±4.30分、59.59±10.42分和34.27±5.91分提高到教育后的53.07±5.78分、41.64±4.55分、68.91±10.31分和37.39±5.36分。在疾病转归方面,干预组的血清丙氨酸转氨酶恢复正常人数、血清乙肝病毒DNA阴转、乙肝e抗源/e抗体血清转换分别由干预前的83.6%、79.1%和22.7%提高到干预后的94.6%、83.9%和41.96%。干预组明显优于对照组(P<0.05)。结论乙肝病友俱乐部是适合乙肝患者的健康教育方式,可调动患者与家属的主观能动性,更好地普及乙肝的防治知识,提高乙肝患者的生活质量。Objective To investigate the effects of health education in the form of patient club on the improve- ment of life quality of and social support for patients with chronic hepatitis B. Methods 222 persons were discovered with chronic hepatitis B during health examination in state-owned enterprises and institutions in Shenzhen. The patients were randomly divided into two groups, namely intervention group （n = 112） undergoing health education in the form of patient club and control group （ n = 110） undergoing conventional health education. Results After one year＇ s health education, the rates of regular drug taking, scientific nutrition, alcohol cessation, rational daily schedule and regular reexamination of participants in the intervention group were 96.43%, 97.32%, 92. 86%, 83. 93%, and 97.32% respectively, all signifi- cantly higher than those of the control group （71.82% , 60. 91%, 45.45%, 63.64%, and 68. 18% respectively, P 〈 0. 01 ）. The improvement degrees of life quality scores in respect of the dimensions of physical health, material life, mental health, and social function of the intervention group were all significantly higher than those of the control group （P 〈0. 01 or P 〈 0. 05 ）. Conclusion Patient club is a good manner to involve the patients and their families in health education, and help improve the life quality of the patients and prevent hepatitis B infection.
