右冠状动脉发育细小的临床评价

摘　　要：目的评价右冠状动脉发育细小(或发育不良)的临床意义。方法本组共收集到选择性冠状动脉造影表现为右冠状动脉发育细小者77例,对其中43例单纯右冠状动脉发育细小者的临床症状和心电图表现进行了分析。结果43例单纯右冠状动脉发育细小者胸闷症状发生率较高,达69.8%(30/43);而其中心电图活动平板试验阳性者仅有6例;目前出院诊断均未单独列出"右冠状动脉发育细小"这一诊断,而多采用了"心脏神经官能症"。结论右冠脉发育细小者较常出现胸闷症状,建议临床采用"右冠脉发育细小(或发育不良)综合征",利于采用针对性药物治疗;鉴于文献有右冠脉发育细小导致心源性猝死的报道,因此对这部分患者做好随访也很有必要。Objective To evaluate the clinical significance of diminutive right coronary artery （or right coronary artery hypopla sia）. Methods To collect 77 cases of selective coronary angiography for the diminutive right coronary artery（RCA）. Among them 43 cases with only diminutive RCA were analysed by clinical symptoms and electrocardiogram （ECG） performance. Results In 43 cases of diminutive RCA,the occurring rate of chest tightness was as high as 69.8%,of which only six patients with positive ECG treadmill test; the discharge diagnosis for ＂diminutive RCA ＂ was not listed alone,and the use of ＂ cardiac neurosis＂ for these cases was very common. Conclusion Chest tightness is a common symptom in people with diminutive RCA, the usage of ＂diminutive RCA syndrome＂ in discharge diagnosis may be helpful for the drug therapy; Some literature has reported the relationship of diminutive RCA with cardiac sudden death,good follow-up for these patients is also necessary.
